Geography next steps • Organizational commitment to ensure all future changes to geographic allocation are in alignment with the final rule • Board resolution designating continuous distribution as the preferred model for distribution • Lung – DSA removed, continuous distribution concept document released • Heart – Board approved new policy, awaiting programming • VCA – Board approved new policy, awaiting programming • Liver – acuity circles policy awaiting implementation • Kidney and Pancreas – public comment proposal out now

  10. Organ Center Kidney Accelerated Placement Concept (KAP)

Organ Center Kidney Offers • Organ Center receives ~2,100 potential kidney donors annually for non-mandatory national shares • Median donor KDPI of 80 • An average of 41 centers (>2300 patients) notified per match • Placement time range - 0.5 hrs to 21.5 hrs • Overall placement rate of these kidneys ~28% in 2017 • Less than 15% placement rate of kidneys with KDPI 80+

Kidney Accelerated Placement Concept • Focus on increasing placement of extremely hard-to-place kidneys through the Organ Center • Using data: • to identify donor “triggers” for accelerated placement • to identify transplant centers that utilize hard-to-place kidneys • Accelerate offers of hard-to-place kidneys to programs more likely to accept them, while continuing to offer to all programs

Transplant Program Qualification Concepts • Accelerating placement by offering first to transplant programs that have accepted a like organ • Identified donor characteristics that differentiate acceptance of marginal kidneys • Based on all kidney transplants performed in the prior two years, updated monthly • Transplant programs qualify if they have transplanted a kidney from a donor with the same or worse characteristics as the current donor • Determined for each specific match in real-time based

Member Quality improvement • Allow members to self-request peer advice prior to policy violation or outcomes flag • Could allow MPSC to offer peer advice • Member, peers, staff would jointly develop a formal change package • Successful outcomes could lead to sharing change package more broadly in the community
