  2. The Physician Leaders’ Network The path to an Integrated Health Care System is paved by Physician Leaders and their health teams who support and champion the value of the Patient’s Medical Home. The Physician Leaders’ Network (PLN) has been designed BY physicians FOR physicians in partnership with: • Alberta Medical Association • Alberta Health Services • Alberta College of Family Physicians The PLN provides free, easy to access essential tools and resources for physicians and their health teams to lead system transformation. The best way to predict the future is to create it – Abraham Lincoln Physicians & Health Professionals
