Population Health: Objectives • Define Population Health • Identify how population health applies to the areas of clinical care, public health, and the community. • Describe the tools that can be used for identification of populations and timely, efficient, evidence-based management of populations across the continuum.

  3. What is Population Health? • In 2003, in an article in the American Journal of Public Health, David Kindig and Greg Stoddart defined population health as “the health outcome of a group of individuals, including the distribution of such outcomes within the group.”

  9. Social Determinants of Health • Conditions in which people are born, grow, live, work, and age. These, in turn, are shaped by the global distribution of money, power, and resources. • Our health is determined in part by access to social and economic opportunities; the resources and supports available in our homes, neighborhoods, and communities; the quality of our schooling; the safety of our workplaces; the cleanliness of our water, food, and air; and the nature of our social interactions and relationships. • The conditions in which we live explain in part why some Americans are healthier than others and why Americans more generally are not as healthy as they could be.

  19. Putting It All Together We have defined population health and looked at the intersection of it with clinical care and public health care. We have explored the importance of Social Determinants of Health. We have looked at multiple ways to evaluate data and manage patients including SlicerDicer, reporting workbench, registries, dashboards, and bulk ordering.
