The completed package including application, essay and recommend- ations must be post-marked by April 1, 2013 Completed application packages can be mailed to: Brandon Junior Woman’s Club Attn: Scholarship Committee P.O. On a separate sheet of paper please list: 1. Community Service 2. College Goals 3. 500 (max.) word essay describing your most rewardingvolunteer experience. The scoring committee will not accept essays about Relay for Life. 4. Attach two (2) letters of recommendation, signed – no copies. 5. Complete this application form. NOTE: Only the items listed should be sent. DO NOT send transcripts, grades, photos or anything extra. Applications will be returned if any one of the five components listed above are missing. When submitting this application, you agree to the following if you are awarded a scholarship: 1. Attend a meeting on May 2ND, 7pm at the BJWC clubhouse (129 N. Moon, Brandon) 2. In December of 2013, write a letter to the BJWC indicating how school is going and your achievements to date. You could be eligible for an achievement award. Mail to: BRANDON JUNIOR WOMAN’S CLUB Attn: SCHOLARSHIP COMMITTEE P.O. BOX 66, BRANDON, FL 33509-0066 Must be post-marked by April 1, 2013. Applications post-marked after April 1, 2013 will be returned to the applicant.

Letters need to be on official school, business, or organization letterhead and person must sign the letter. If individual writing the letter is not associated with a school, business, or organization, they must include their name and address on the letter. Send original signed letter. Copies will not be accepted. • Attach the completed application form to the above requirements and submit on or before April 1st.
