    4. Health Need in North Somerset In North Somerset two wards fall in the top 10% of deprived areas in the country and five fall in the bottom 10% deprived areas in the country. Weston Super Mare South and Weston Super Mare Central are the most income deprived wards in North Somerset The gap in life expectancy between the most and least deprived areas has always been present but in recent years appears to be growing.

    5. Gap in Life Expectancy At the ward level there is a 13 year gap between the highest and lowest life expectancy. Weston Super Mare Central has the lowest life expectancy (72 years), falling in the bottom 5% in the country.

Chronic Liver Disease In WSM South and Central deaths due to chronic liver disease were significantly higher than would be expected and made up 2% of all deaths compared to only 1% in North Somerset. Across North Somerset there were 102 deaths due to this condition, of these 46 (45%) occurred to residents living in either WSM Central (16 deaths), WSM West (13 deaths), WSM South (11 deaths) or WSM Clarence and Uphill (6 deaths).

    14. History In 2010-2011 NHS North Somerset withdrew funding from the Community Safety Drug Action Team to manage Alcohol within the PCT under one contract. This contract was awarded for one year to a local preferred provider The specification has been drawn up with engagement from our multi stakeholder Alcohol Strategy Group, in liaison with The South West Lead for Alcohol Services and a stakeholder morning held in Weston Super Mare The PCT had approached the Community Safety Drug Action Team for the tendering process to be completed in conjunction with the tender for drug services.

Function of specification The function of the North Somerset Alcohol Specification is to provide interventions through a mixture of direct access and appointments that minimise barriers to entry. The services should provide a single service assessment gateway to appropriate alcohol treatment, identified through AUDIT screening, assessment.

    17. Expectation of the service Offer brief 1 -3 interventions, to reduce alcohol-related harm and build motivation to change, to hazardous and where appropriate harmful drinkers. Structured psychological interventions, integrating solution focused therapy cognitive behavioral therapy & motivational enhancement therapies to promote abstinence or controlled drinking, reducing alcohol related harm in harmful or moderately dependent alcohol misusers. Transition management of severely dependent drinkers identified through assessment process as requiring detoxification at Tier 3. The service should work in Liaison through partnership protocols with other agencies and professionals in criminal justice, health & social care fields, including probation, courts, housing, and employment support Maximise treatment complete, alcohol free outcomes and minimise those re-presenting to the treatment system via relapse prevention programmes.

    18. Expectation of the service Transition management of severely dependent drinkers identified through assessment process as requiring detoxification at Tier 3. The service should work in Liaison through partnership protocols with other agencies and professionals in criminal justice, health & social care fields, including probation, courts, housing, and employment support maximise treatment complete, alcohol free outcomes and minimise those re-presenting to the treatment system via relapse prevention programmes. Consideration should be given to the length and frequency of intervention offered to maximise effectiveness and maintain engagement

    21. Terms and conditions The contract once awarded will be monitored under the terms and conditions of The DOH Community Contract with regular performance meetings. As part of the implementation of the Alcohol Strategy Action Plan it will also be led and monitored by the North Somerset Alcohol Strategy Group. The specification includes the outcomes expected through delivery of the service and describes the way delivery and performance will be monitored.
