  4. HOW WE OPERATE The Board and its staff assures the public and those it licenses that there is a balanced, sensible approach to regulation, with the safety, health and welfare of the public being our primary concern.

  5. BOARD COMPOSITION The Kansas State Board of Healing Arts is comprised of 15 members appointed by the Governor and serve a four-year term. • 12 licensees • 5 M.D.s • 3 D.O.s • 3 D.C.s • 1 D.P.M. • 3 public members

  8. APPLICATION PROCESS • All application forms are available online at http://www.ksbha.org/forms.html • Application Fees $60 + $25 for temporary permit. Check, Money Order, Visa, MasterCard, Discover or American Express

  9. APPLICATION PROCESS • Be sure to answer all questions • Request an official and final transcript • Certification by school • Signature and school seal or notary seal • Photo: Wallet size, head and shoulder • Picture taken within 90 days

  10. APPLICATION PROCESS • Sign and notarized seal for Oath • Sign and notarized seal for Release • Documentation and explanation to any “yes” answers to #12 • Verification of any license, registration, &/or certification in any health field.

  11. APPLICATION PROCESS Application status can be verified by phone or e-mail but please allow at least 2 weeks after sending in the initial information.

APPLICATION PROCESS • Initial Review • A review of documents to verify if all required documents have been provided and are complete • A letter and/or e-mail will be issued to notify applicant of any missing documents or needed corrections • As documents come in, periodic letters will be mailed to advise applicant of the progress

APPLICATION PROCESS Common Problem Areas • Photo: wrong size or inappropriate • Not answering the NPI question • Not answering the activities for the last 5 years • Not sending in documentation or explanation to “yes” answers on the questions in #12 • DO NOT provide info that is not required (e.g. diplomas)

  14. APPLICATION PROCESS • Final Review -All documents are reviewed again • Results • Request for additional information • Clarification of information • Referral to the Legal Department • Approval of temporary license (if applicable) • Valid for 180 days • Approval of license

LICENSE RENEWAL INFO • License renewal will be required of all receiving a permanent license prior to June 1 • Annual renew between August 15th - September 30th • The license expires September 30th • The current renewal fee: $50.00 • The late renewal period is October 1st through October 31st • The current late fee: $55.00 • Renew online or by paper renewal form • Wallet Card (certificate of renewal) will be mailed

  16. CONTINUING EDUCATION • During the 12-month period immediately preceding the license expiration date, a minimum of 12 hours of CEUs are due. • If licensed less than 1 year no ceus are due
