  • Print the test sheet at the end of this Module as “current slide”.

  • Conduct this Moduleand give the WRITTEN TEST (100% passing grade required) to all of your members AT YOUR NEXT MEETING,

Is this module mandatory yes

  • NM Wing Command now requires that Module with 100% passing grade: 100% of ALL NM CAP members complete this required Form 78 Training Module and Test on a yearly basis.

  • Failure to complete this training may preclude amember from participating in ANY CAP activity (including driving, aircrew, Senior or Cadet activities, etc).

Why is this module mandatory

  • During 2007 through 2009, NM Wing was found to be Module with 100% passing grade: non-compliant with CAPR 62-2, which requires an online Form 78 Mishap Report within 48 hours of any damage to CAP property, or personal injury.

  • NM Wing MUST comply 100%with CAPR 62-2 Mishap Reporting requirements. Failure to comply is not an option.

  • Only you, as a CAP team member, by timely Form 78 Mishap Reporting, can ensure that our NM Wing remains compliant with CAP regulations.

Case 1 blanik glider

June 2007: Module with 100% passing grade: During the 2007 glider encampment, a CAP glider was substantially damaged during a hard landing, but the resulting tail cone dent was not reported per regulation.

July 2008: One year later, during the annual inspection, the damage was found to involve critical internal tail structures as well. The estimated cost of repair was $10,000. The Form 78 was still not filed until November 2008!!

Case #1 Blanik Glider

Problem: Aircraft Damage not reported.

Case 2 cessna 182

February 2009 Module with 100% passing grade: : A metal tear and dent to the trailing edge of the horizontal stabilizer of N737NM was noticed but not reported. The AC was not inspected by an A&P, yet the AC continued to be flown.

May 2009:During the NM Wing Compliance Inspection, the damage was noted, and the AC was grounded until an inspection by an A&P was completed. The Form 78 was not filed until May 2009!

CAPR 62-2 MISHAP REPORTING AND INVESTIGATION: Module with 100% passing grade:

requires by regulation that any and all incidents involving damage to property (aircraft, vehicles, assets, equipment) or personal injury be reported within 48 hours using the online Form 78 utility.

CAPR 62-2 has tables that define what constitutes a Mishap. Found at “www.nmcap.org” , click on Safety.

When must a form 78 mishap report be filed within 48 hours

  • --Anytime Module with 100% passing grade: there is damage to CAP property (Aircraft, vans, equipment, buildings, hangars, etc) which requires repair or replacement.

  • --Anytime there is an injury to a CAP member which requires some form of medical attention.
