    1. Ultimate Football Affective Test By: Brent Greer

    2. Affective Test Purpose: To test the students on non-skill abilities before the game, during the game, and after the game. Ex: Sportsmanship & Fair Play Instructions: I need everyone to split into two equal teams. We are going to play a game of ultimate football. While playing you are to meet the 5 criteria.

    3. Modifications Modifications: If someone is injured they will keep score of the game. Absent: If the student provides valid excuse they can take the cognitive test. Weather: If it rains we will play inside on the gym floor.

    5. Scoring Scoring Procedure: Place a check in the box. Scoring Criteria: Each question is worth 1 point for a total of 5 points
