    4. What is Gift of Life – Arizona, Inc. ? Founded in 1985 as a project of the Rotary Clubs of Arizona Districts 5490, 5500 and 5510 A nonprofit organization, 501 (c) 3, providing a means for needy children from around the world (including Arizona) to have life saving corrective surgery for congenital heart defects Three-fourths of the directors shall be Arizona Rotarians from Rotary Districts 5490, 5500 and 5510 25 Board Members Allowed 16 Rotarians on GOL-AZ Board 5490 = 5 5500 = 2 5510 = 9 2 Non-Rotarians on GOL-AZ Board Pays a stipend of $5000 to the hospital for all procedures needed per child Doctors and other professionals donate their time and skills Host families provide transportation, food and lodging for the patient and parent

What’s Happened…1985? 203 Children helped 10 per year average 20 Countries Bolivia (9) Dominican Republic (5) El Salvador (3) Ethiopia (1) Guyana (1) Haiti (1) Honduras (4) Indonesia (1) Mexico (125) Nairobi (1) Nicaragua (9) Nigeria (1) Pakistan (1) Philippines (1) Russia (1) Rwanda (1) South Korea (8) Sir Lanka (8) Trinidad (1) USA (21)

    6. How can assistance be provided? Financial donations: Arizona Rotary Clubs, Rotarians, private donors, small companies and corporations Host Families: Rotarians & Non-Rotarians Provide transportation, food and lodging for the patient and parent Recommend “needy” Children: Arizona Rotary Clubs Rotary Clubs in other countries

    7. YOUR HELP IS NEEDED!!! Club Budget to include becoming an Annual Supporter based on club size Have Gift of Life-AZ as a club program Provide Gift of Life information for club membership when supplied

    10. 2006-2007 GOALS Finish the 24 needy children awaiting surgery 9 from the Phoenix area Finish the installation and training of the ICU rooms at Public Children’s Hospital in Hermosillo, Mexico R. I. Matching Grants supported by several Arizona Rotary Clubs, 3 Arizona Districts, and Mexico Rotary Clubs Should allow an additional 50+ surgeries per year Want to achieve 100 surgeries per year Requires a budget of over $500,000.00/year
