What is Back Up Programme? • Aviva have launched a new innovative back pain clinical case management service for members with back or spine problems with <Company Name> employees and their families. • What is case management? • “..an active process that involves the assessment of an individual and the development and implementation of a tailored rehabilitation programme”. • Evidence based approach & outcome focussed on “delivering the right treatment at the right time at the right cost for the right outcome”. • Independent expert case management from our rehabilitation partner HCML

Back Pain • Over half of Irish workers experience back pain each year • Over a quarter of non-fatal injuries in Irish workplaces are attributable to back pain. • Up to 80% of the adult population will suffer from significant back pain at some point in their life. • 7 million lost working days per year due to Musculo-Skeletal Disorders (MSD) • €348 million cost in lower back pain disability payments • in 2002 • *Source: Fit for Work?   4. Back- Up Teams • There are three teams of case managers who deal with • Back- Up cases All members of the team have a professional • health qualification e.g. • Sports Rehabilitation • Sports Therapy • Bio-kinetics • Physiotherapy

Back-Up Customer JourneyOverview • Employee phones Aviva on 1890 717 717 • Aviva book in the employee with HCML • HCML phone employee to have initial discussion. • HCML may recommend the employee attends a Physiotherapist for consultation • The Physiotherapist may then recommend a course of Treatment • A once of fee of €50* is required at the 1st treatment session • – this is to encourage them to complete the treatment programme • All other treatments are Free of Charge • *€50 fee not payable under day to day claims
