  6. Just Checking The Just Checking system allows people with dementia or memory loss, to continue to live independently. It monitors a person in their home, and provides a chart of activity via the internet.

  7. Set up and Installation • Briefcase – portable, self contained – only need a power point in property. • Simple to explain – good support material. • 20 minutes to install, 5 minutes to remove. • Basic IT skills. • Secure sharing of information via internet • Cost effective £14 per week inc over 3 years. • Can be used with consecutive users across the year.

Background • Introduced in Cannock Chase in June 2006 with JJ 82 year old female with dementia. • Lived alone fiercely independent, no insight into her advancing illness, her neighbours and community very concerned. • High pressure to admit to care. JJ loved her home – distant family supported her right to take risks. • Just Checking highlighted changing risks as dementia advanced. Used with MDT meetings. • Care package involved Family, Friends, Shopkeepers, Police, Chapel, School, Third Sector CMHT and Social Care and Health.

JJ was supported at home for a further 6 months. • Promoted across County – Innovations award. • Pilot case led to 12 kits being purchase for use across County in Autumn 2006. • Of 85 installations County wide to date, 37 within Cannock Chase. • Achieved this amidst major organisational change and uncertainty.

Early learning • Identify risks in own environment. • Avoid trauma or residential assessment or overnight sitters. • Challenge subjective information. • Consider which resources are appropriate. • Understand the impact of services. • Evaluate the outcome. • Promote the right to live safely within community. • Cost effective.

Costs • 3 year contract – each kit costs £14 per week – equal to 1 hour of domiciliary support. • We are using our kits 9-10 times in a year • Average assessment cost – £50-£70 for 4-5 week period. • One night’s overnight sitter would cost £150+ • One week in long term care costs Department £250+

  15. 1 • Female 68, Dementia, Vulnerable in own home • Referred for long term care by GP • Supportive daughter and Landlord • Just checking installed • Hospital admission – SW and Daughter advocated return to Community, challenged ward staff. • Transition plan - enabling care package monitored by Just Checking. • Re-housed – long term care package • GH living in own home in Community • Saving to Dept £50 per month.

  16. 2 • Male 75, dementia, living alone after wife dies. Supportive family live locally. • KB wishes to live in own home but family considering moving him into extension funded by DFG – cost £15,000+. • Just Checking highlights his restless behaviour within home care package and telecare equipment used to reduce risks. • Family decide they could not cope with this activity in own home. • Care package is £500 per month cheaper than cost of care home • DFG application cancelled.

  17. 3 • Female, 71, Dementia, Living alone. Up all night. • Supportive but worried family, Considering long term care. SH wants to live at home. • Just Checking providing detail of activity • Telecare sensors fitted - now finding way back to bed at night. • Care package adjusted and monitored • SH still at home 70 weeks later & Carers are less stressed • Cost of care package now saving to Dept £300 per month compared to long term care costs.

  18. Promotion and training • Identified local champions – owned within locality. Cascade training with support from Just checking. • Encouraged through team meetings and supervision, peer support. • Programme of awareness sessions attended by all partner agencies. Use the of CSIP Telecare DVD and demonstration of equipment in our local smart suite. • Promoted at partnership events. • Encouraged and fast tracked referrals from partner agencies. • Support from Councillors, Director and DMT

  20. CCDC Housing pilot • Proactive staff team - Owned and installed by Scheme managers. • Used in supported Housing to monitor and support tenants at time of transition. • Promoted within tenants forums. • Reassurance to residents and carers. • Early alert to difficulties - earlier referral.

CCDC Housing PilotFeedback from Sue Plumbe • Residents were complaining of a flat door banging and someone trying to open flat doors during the night and in the early hours of the morning.   • District Nurse was concerned about Mrs A who had to have daily injections for Diabetes and had very differing blood sugar levels she wondered what time Mrs A was having breakfast, as this lady has memory loss this was difficult to establish.  Carers concerned about Mrs A that seemed to be permanently tired and listless. •  The system was very easy to fit and Mrs A and family had no objections to having it fitted for a trial period.

  22. The Just Checking System has proved to be a great success as it monitored the movements of Mrs A and it showed she was opening her door several times a night also going out of her flat at night and early hours of the morning for as long as two hours . • It showed that Mrs A was in the kitchen in the early hours of the morning so was having her breakfast any time from 3am and not going to bed for longer than one hour at night.   • The outcome of this test was that the other residents are not as concerned. the District Nurse now can understand why the sugar levels vary and we understand why this lady is so lethargic in the day time and is often asleep in chair when I do my checks. • Now looking at ways to help her.

  23. What next • Access team, Enablement team and Transition workers. • Joint training Feb 7th 2008 to develop maximum use of the additional 7 kits in Cannock Chase Locality. • Personalisation agenda • Unique Care and Unique Care plus
