  2. OLGU • 66y E • Şikayeti: sol bacakta yürüme ağrısı • Özgeçmiş: 5 yıl önce CABG • 4 yıl önce sol tarafa femoropopliteal bypass • sağ bacak diz altı ampüte • Komorbidite: DM, HT, SVO, 40 paket yıl sigara • Alt ekstremite RDUS ve BT anjiografi

  3. KDC; sol femoropoplitealgrefti revize etmeden önce sol iliak darlığın endovasküler yolla tedavisini istiyor. ENDOVASKÜLER TEDAVİ PLANI? 1. İpsilateralendovasküler tedavi 2.Kontrlateral 3.Aksiller 4. Aortofemoral cerrahi bypass

  4. 1.Distale stent ekleyelim 2.Sadece PTA yapalım gerekirse stent 3.Vazospazma bağlı perlinganit yap ve bekle 4.Haliyle bırak

  5. 1.İpsilateralden kaplı stent 2.Kontrlateralden kaplı stent 3.Rüptüre segmentte balonu şişir ve durmasını bekle 4.Balonu şişir ve cerrahlara haber ver

  6. KAPLI STENT + MANUEL KOMPRESYON

  7. MANUEL KOMPRESYON YOK 1.İkinci bir kaplı stent 2.Balon şişir bekle 3.Over size çıplak stent yerleştir.

  8. 20.dk Balonu şişirdikten 10dk sonra

  9. POSTOP 1. HAFTA KONTROL KONTROL BT ANJİOGRAFİ
