  9. HOUSEHOLD QUESTIONNAIREITN MODULE Preparation • Contact National Malaria Control Programme • Identify all brands of treated mosquito nets • Obtain photographs or description of brand logos to serve as aids in field (sometimes difficult to identify brands by observing)

  10. MALARIA MODULES (continued) Add ITN module to household questionnaire prior to the salt iodization module.
