  2. Who is Eligible? • General Managers – Family Coverage • Co-Managers – Single Coverage • Sr. Assistant Managers – Single Coverage • Assistant Managers – Single Coverage • Shift Managers - Mini-Med Plan • Crew Leaders – Mini-Med Plan • Safety Marshalls – Mini-Med Plan Package I Package II

  3. Package I

  4. Medical Insurance Central Reserve Life Deductibles: • Single – $1,100 • Family – $2,150 • No-Co Pays • When an employee has met their deductible they are covered at 100% when using in-network providers. Out of network 80% • Cost of premium is based on employees age and gender. • Deductions made each pay to cover the 20% employee portion of premium.

  5. Medical with HSA • Network provider listing is available at www.worldinsco.com/crl or by calling (877)-575-4207. • Health Savings Account (HSA) • Company funds 50% of deductible, deposited into the employees HSA account each pay period. • Employee funds the remaining 50% of deductible, deposited into the employees HSA account each pay period.

  6. Medical with HSA • Funds deposited into United Missouri Bank (UMB) • Tax-free • Employee owned account • Use funds to pay for medical expenses • Terminated employees take the account with them • HSA account information is available to the employee at (888) – 367-6727

  7. Medical with HSA You will receive the following with this coverage: • Certificate of coverage • Booklet explaining coverage • Medical Insurance ID Card(s) • Prescription Discount Card • Lab Discount card • Debit Card • Starter Checks

  8. Medical with HSA Employees who chose not to take the Package insurance, they will receive an annual allowance: • Single Coverage: $1,000 • Employee/Spouse: $2,000 • Family Coverage: $2,500 There is a 60 day waiting period for all employees beginning the date they become eligible.

  10. Dental Insurance The Standard Company Deductibles: • $50.00 – this is waived on Type 1 procedures • Maximum coverage per year, per person is 1,000

Dental Insurance Three types of procedures: • Type 1 – include but are not limited to cleanings, exams and x-rays – Covered 100% • Type 2 – include but are not limited to sealants, oral surgery-simple extractions, root canals – Covered 80% • Type 3 – include but are not limited to space maintainers, anesthesia, oral surgery – complex extractions, gum disease-surgical, crowns, dentures, bridges – Covered 50%

  12. Dental Insurance Three types of procedures: • Open access plan – You can go to any doctor you wish. Whether they are in the network or not your coverage will be the same. • Network provider listing can be found at www.standard.com or by calling (800)-633-8575.

  13. Medical with HSA You will receive the following with this coverage: • Booklet explaining coverage • Dental Insurance ID Card(s)

  15. Vision Insurance Vision Service Plan Coverage: • $20 co-pay • Exam – receive one exam every 12 months • Glasses – lenses 100% covered, every 12 months • Frames - will pay up to $120, every 24 months • Contact Lenses – up to $120, every 12 months

  16. Vision Insurance There is no ID card needed for your vision coverage. Simple tell your doctor you have VSP. Network provider listing can be found at www.vsp.com or by calling (800)-877-7195.

Disability Guardian Insurance • All Managers eligible for Package I coverage are eligible for this short –term disability coverage at no cost to them. • Benefits pays 60% of your weekly wages up to $750 • Benefits begin on the 15th day of an accident or illness (provided it is not work related) • You will receive the benefit for a total of 13 weeks

  19. Enrollment Procedures

Enrollment Guidelines Package I • Employees become eligible when hired or promoted into position of Assistant Manager or above. • Applications must be completed and faxed to office within the first 30 days of eligibility. Otherwise, any pre-existing medical conditions will not be covered for a period of 18 months from the start of coverage. • Coverage should begin on the first day of the month following a 60 day waiting period.

Mini-Med Medical Nationwide • Two plans to choose from based on manager needs • 100% of premiums are employees responsibility (payroll deduction) • No deductibles or co-pays • No Pre-Existing Conditions Limitations • Use any doctor or hospital • Coverage include: • Doctors visit benefit • Emergency Room • Prescription Discounts

  24. Mini-Med Dental The Standard Insurance Company • 100% Coverage for Preventative Care – No deductible. • 80% Coverage for Restorative procedures - $50 deductible • 50% Coverage for Major Restorative Procedures - $50 deductible • Maximum payout per covered member is 1,000

  25. Mini-Med Vision Vision Service Plan (VSP) • $15.00 Co-pay for exams every 12 months • $25.00 Co-pay on materials: • Lenses every 12 months • Frames every 24 months • Contacts every 12 months – no co-pay

Enrollment Guidelines Package II • Employees become eligible when hired or promoted into position of Safety Marshall, Crew Leader and Shift Manager • Applications must be completed and faxed to office within the first 30 days of eligibility. Otherwise, they will have to wait until next open enrollment or qualifying event. • Coverage should begin on the first day of the month following enrollment.
