  3. Application Process • 1. Review PPO Provider Directories and if provider is NOT on the PPO list, decide if PPO is appropriate. • 2. Download and print a PDF of an application on the insurance website, or complete your application entirely online. • 3. All employees must re-enroll in health insurance, and make a choice between tradition or PPO. • 4. Complete paper application and turn into the agency payroll office by may 28th at 5:00 PM • 5. The agency payroll office will enter your information via IPOPS. • 6. The agency payroll offices will receive a limited number of paper application. Supplies are limited.

  4. Dear Mr. Krebs, After reviewing your presentation there were a few minor changes that I would like to address. On slide one I added a background that included color to make it look more appealing and I also added who the author of the slides were on the front page. On slide two I added a background with colors that coordinated with the cover background as well as added bullets to make it look more organized and easier to read. On slide three I added the same background as well and I also added bullets with numbers so it makes it organized, makes the steps clearer and I added a title so the audience knows what the topic is about. By adding number I believe that it makes the information look more presentable and easier to follow. I hope these changes helped you out. If you have any further questions feel free to contact me.
