INTO VIRGINIA ICPC 100A signed by birth parent (4 copies), showing the birth parent as having planning and financial responsibility 2 complete packets Cover letter Consent document signed by the birth parent to reflect that the birth parent knows the prospective adoptive parents (adoptive parents’ full name is given) and based on this knowledge chooses to place her child for adoption with this family Documentation addressing how the legal rights of the birth father (and legal father if applicable) will be handled Legal risk statement (if needed) Birth mother and birth father social background and medical history Child's complete hospital records (if infant) or current medical records if older child) Adoptive parent(s) home study by licensed Virginia child placing agency.
