The medical assessment form is a supplemental form only • The medical assessment was designed for school nursing staff in case additional information was needed for documentation. • The medical assessment does not have to be filled out with each I/A form • The medical assessment does not replace the I/A form

FAQ’s • Who should fill out the I/A report form? • When should an I/A form be submitted? • Why is there a medical assessment form? • There is no red flag indicating a digital signature is needed, what do I do? • Why after filling out the form and sending to my Admin is the form blank? • How does my Administrator retrieve his/her password?

  6. To remove a digital signature in Adobe you must delete the signature file from the computer. The file is located in the user’s folder under Document and Settings. • Here’s the path to the file: • C:\Documents and Settings\username\Application Data\Adobe\Acrobat\10.0\Security • Username will be replaced the person’s login name. • The file to be deleted is the pfx file. • Once the file has been deleted, you will be prompted to create a new digital signature.
