  2. AGENDA • What’s New and Changing for • 2012: • Health Insurance Carrier Options • Pharmacy Benefits • Dental Insurance Premiums • Health Saving Accounts Limits

What’s New – Health • Previously, HMO members could choose from several providers. • Department of Management Services (DMS) - solicitation for the HMO Medical Plan and Pharmacy services. • DMS awarded contracts for HMO services on a county-by-county basis. Two (2) HMO providers for Miami-Dade, Broward and Palm Beach & St. Lucie counties will now be • AvMedand Coventry Healthcare.

  4. What’s New – Health • United Healthcare (UHC) will no longer be an HMO provider for these counties. • All HMO members are required to elect a participating HMO provider for their county of residence/work, should they wish to continue in an HMO plan. • Default medical plan is Blue Cross/Blue Shield PPO plan (no change to current members).

  5. What’s Changing - Pharmacy • New Prescription Benefits Manager (PBM): Medco (HMO & PPO Health Plans) • New ID Cards for Medco PBM • Walgreens will no longer be a participating pharmacy • http://www.medco.com/sofrxplan • 1-877-531-4793
