  • Death of a student

  • Not an aid recipient

  • Only eligible to receive unsub funds

  • Verified by another school (need a letter from that school)

  • Selected for verification post enrollment and disbursement already occurred

Spouse of an independent student

The spouse has died

Spouse is mentally incapacitated

Spouse resides in a country other than the U.S. and can’t be contacted by normal means

Who cannot submit a Tax Return Transcript?

  • Married filing separate tax returns

  • Unmarried and living together

  • Amended Tax Return filed

    • A signed copy of original tax return or tax return transcript AND

    • A signed copy of IRS Form 1040X that was filed with the IRS

Reporting Results for V4 &V5 Verification

  • Schools must report the results of verification for those students selected in the V4 or V5 tracking groups

    • Students selected by schools and not in V4 or V5 are not to be reported

  • Reporting must be done within a short time after the end of the 2014-2015 aid year.

  • Results may be submitted individually on CPS online, or beginning April 2014 by uploading a flat file to the CPS web site.

Unusual Enrollment History

UEH Flag 2 and Comment Code 359

  • Must review the last three years awards to determine if a Pell grant at the institution was received

If yes, no additional action is required

If no, using information from NSLDS, the institution must identify all schools where

the student received a Pell Grant for the last three award years and determine

if the student earned academic credit at each of those schools during the

award year for which the student received a Pell grant
