  2. Welcome to PN 152 Clinical • You are in your final clinical experience here at Concorde. Congratulations! This is an exciting term with different clinical experiences. You will have half of the timesplit with an instructor led leadership and other half one on one with a CTA (Clinical teaching associate or mentor) at a clinical site. • All the forms, information and paperwork you need before, during and at the completion of your clinical can be found in the packets and onSharepoint. You can print if misplaced. • If you have questions please ASK! • Every student is required to view this Power Point.

  7. Sharepoint Documents Continued • Preceptorship Instructions – Highlights • CallMikefirst thing when you get to your preceptor site everyday. (No call could mean being marked absent). • Attendance – • You must attend every day (includes leadership and clinical enrichment ). • If you are 30 min. late, it counts as absence. • 3 tardies equals an absence • If you are absent, your time will be made up where it was missed • More than 2 absences may result in withdrawal from the program. • Meet with your Clinical Teaching Associate (or alternate) first thing daily.

Sharepoint Documents Continnued • Preceptorship Instructions – Highlights Continued • PAPERWORK • Journals due weekly for all students each week • Letter of Agreement signed by student, CTA and instructor, due the first week of preceptorship • Clinical Log record hours daily and have CTA sign them • Evaluations due at midterm and end of term

Sharepoint Documents Continued • Final Preceptorship Schedule • Final Leadership/Clinical Enrichment Schedule • Clinical Enrichment Folder – no documents at this time • A few site specific folders such as MOHS (OHSU), corrections, etc. Call your site to ask for specific start time, person to report to, uniform requirements, and unique site requirements

  11. Sharepoint Documents Continued • PRECEPTED CLINICAL EXPERIENCE REQUIREMENTS • Students should arrive prior to the start of your shift . Arrive early the first day to allow time to find your location. • At no time will a student be allowed to leave a clinical site early unless there is an emergency or the student becomes ill during the clinical day. Contact your instructor prior to leaving clinical! • Students are not allowed to attend clinical if they are ill • Report an absence atleast 1 hour before the start of the clinical shift to your instructor and site.

Sharepoint Documents Continued • EVALUATION DOCUMENTS • Student Evaluation of Precepted Clinical Experience • Completed by the student • Preceptor Experience Evaluation • Completed by the CTA • Term 5 College Evaluation • Standard clinical evaluation • Completed by CTA • ALL PAPER WORK IS DUE PRIOR TO THE LAST DAY OF MID-TERM AND END OF TERM • YOU WILL ALSO BE HAVING VISITS ON SITE FROM YOU INSTRUCTOR

  14. Sharepoint Documents Continued • WEEKLY JOURNALS • Must be completed each week • Must be turned to your preceptor, leadership or clinical enrichment instructor • Must be signed • Confirms your attendance
