    3. Early Intervention Transition Indicator 8C Percent of all children exiting Part C who received timely transition planning to support the child’s transition to preschool and other appropriate community services by their third birthday including:

    4. Early Intervention Transition Indicator 8C IFSPs with transition steps and services; Notification to LEA, if child potentially eligible for Part B: and Transition conference, if child potentially eligible for Part B.

    5. Part B Indicator 12 Preschool Transition Percent of children referred by Part C prior to age 3, who are found eligible for Part B, and who have an IEP developed and implemented by their third birthdays.

    9. 6-9 Months Prior to Child’s 3rd Birthday Provider(s) Participate in transition planning, as appropriate Continue to document ongoing assessment Administrative Unit Review child and family information to plan for Part B eligibility evaluation Participate in transition planning as appropriate Support family in understanding preschool service options

    10. 3-6 Months Prior to Child’s 3rd Birthday Service Coordinator Coordinate with AU in arranging the Part B evaluation With parental consent, send a complete copy of the child’s current IFSP, including updated on-going assessment data and notes, to the AU Update the transition plan, as appropriate Administrative Unit Review all current information on the child in order to plan for Part B eligibility evaluation Provide written notification with the date, time, and location of the evaluation to the family & service coordinator

    11. 3-6 Months Prior to Child’s 3rd Birthday Administrative Unit At the time of the evaluation obtain written consent and provide the family and explain their procedural safeguards Conduct the Part B eligibility evaluation and assessment

At Least 3 Months Prior to Child’s 3rd Birthday Service Coordinator Ensure transition conference held Provide notice of transition conference to AU and any other appropriate participants Share updated info with AU, as appropriate Implement transition plan as appropriate Provider(s) Ensure current ongoing assessment information is available Participate in transition conference, as appropriate

    13. At Least 3 Months Prior to Child’s 3rd Birthday Administrative Unit Complete Part B eligibility determination If eligible, participate in transition conference to review preschool options with family

By Child’s 3rd Birthday Service Coordinator Complete all requirements listed in the transition plan As desired by parent, participate in IEP Meeting Provide exit status data for CCMS Provider(s) Ensure completion of exit data in Results Matter online system Complete all requirements listed in the transition plan, as appropriate and identification of any new issues

By Child’s 3rd Birthday Administrative Unit Complete all requirements listed in the transition plan, as appropriate Ensure Part B eligibility determination process has been completed Develop child’s IEP, including identification of start date for Part B services Determine need for ESY services, as appropriate

Transition Conference Must be held at least 90 days prior to child’s 3rd birthday LEA/AU must be invited for children potentially eligible unless the parent declines Should include review or development of transition plan, including identification of steps and services necessary to support a timely transition

Transition Plan vs Transition Conference Transition plan can be developed at any point in the IFSP process appropriate to the child and family’s needs, but no later than 90 days prior to the child’s 3rd birthday Transition plan must include steps and services necessary to support a timely transition The transition plan can be updated as appropriate to the child and family’s needs

    18. Transition Plan vs Transition Conference The transition plan should be documented on the Transition Plan page of the IFSP The transition conference is a meeting to discuss transition options and do the final plan to ensure a smooth and timely transition

Transition Plan vs Transition Conference For children potentially eligible for Part B Preschool services, the LEA must be invited to the transition conference, unless the parent declines consent When the parent declines consent to invite the LEA to the transition conference, attempts should be made to conduct the conference with other community options for transition

    20. Community Strategies Establish an Interagency Operating Agreement to identify roles, responsibilities and expectations for partners regarding transition from EI to Part B Review system, challenges, data periodically
