  4. Required NMSEF Forms for All Students • All forms are interactive PDFs • Student Entry Form – Cover Sheet • Student Checklist - Form1A • Adult Sponsor Checklist – Form 1B • Approval - Form1

  5. Many students will need these forms: • Qualified Scientist – Form 2 • Risk Assessment - Form 3

  6. Some students (high school only) may need these additional forms: • Human Participants - Form 4 • Human Participant – Informed Consent • Regulated Research – Institution Form 1C • Potentially Hazardous Risk – Biological Agent Form 6A

  7. Students will NOT need this Abstract form for NMSEF • When students are submitting the abstract, simply use a word document. At CHECK-IN, the night before science Fair, 3 copies will need to accompany the student. • ONLY the PROJECT TITLE and CATEGORY should be on this abstract, no student name.
