  2. New Coverage Effective January 1st, 2012 ELEA and Reliance Standard are pleased to announce the addition of Critical Illness and Accident insurance effective 1/1/12. Each active full-time Employee working at least 20 or more hours per week will be eligible to take advantage of these voluntary coverages. During the following slides you’ll be given an overview on both coverages Please review your policy documents for additional information The information given in these slides does not represent a contract. Please refer to Your policy document for details

  3. How does Critical Illness insurance work? Critical Illness insurance is designed to pay a lump-sum benefit at the diagnosis of a covered critical illness. Pays the benefit directly to the insured so you can spend the money as you see fit and stay focused on getting well. You can choose the level of coverage most appropriate you and your family.

  4. What is covered? Key Advantage: Covered Critical Illnesses are divided into 3 different categories, This allows for the insured to Collect a benefit multiple times.

  5. Why are there different categories? Collect multiple benefits from the same category You can collect 50% of your benefit if you suffer from a 2nd diagnosis from the same category. This must occur no sooner than 18 months after the original diagnosis. Collect a Benefit twice from a different category You can collect 100% of the benefit amount if you suffer a diagnosis from a Different category. This must occur no sooner the 6 months after the original diagnosis Each category has a 200% lifetime maximum allowing the coverage to pay up to 6x the chosen benefit amount.

  6. Wellness Wellness Benefit An insured is eligible to receive a $50 benefit every 12 months upon the completion of a wellness screening. Some examples of accepted screenings are below $10,000 of Family Coverage for a 35 year old = $162.00 Minus (3) $50 Wellness Benefits - $150.00 Total Cost per Year = $12.00

  7. Included Features Portability Allows the insured to continue coverage if he/she ceases to be eligible. Coverage must be in force for at least 12 months prior to exercising portability. Pre-Existing Limitation (12/12) Conditions diagnosed or treated within 12 months of the policy effective date will be covered after coverage has been in place for 12 months. Guaranteed Issue Coverage (Special One Time Offer) Employees and Spouses who apply for coverage under $15,000 will not have to answer medical questions or be medically underwritten.

How does Accident insurance work? • Accident insurance is designed to pay a pre-defined benefit based on an accidental injury or a qualifying event. • Pays the benefit directly to you so you can choose how the money is best spent. • You can choose the level of coverage most appropriate you and your family. • There are no health questions to answer, enrolling is easy. • No Network, you can choose your own treatment. In 2005 over 30 million people sought emergency treatment for an accident and Almost 3 million were hospitalized.

  10. What is covered? Voluntary accident insurance will pay a benefit for over 100 different accident related injuries or treatments. These benefits fall into one of these categories. Please refer to your brochure for the full list of occurrences Key Advantage: Your accident insurance has no dollar limit, regardless of how often you utilize your benefit.

  12. Wellness Wellness Benefit An insured is eligible to receive a $75 benefit every 12 months upon the completion of a wellness screening. Some examples of accepted screenings are below Employee and Spouse coverage = $454.20 Minus (2) $75 Wellness Benefits - $150.00 Total Cost per Year = $304.00
