Policy and Procedure(pages 4-6) • District policy and procedure is based on Washington State law • The law must be followed when school staff take any responsibility for a student’s oral medication • Must be trained and supervised by a registered nurse • All requirements must be met to administer medication at school

  4. Intake (Receipt) of Medication(Page 7) 1. FORM COMPLETE • Student info (fill out top of log both sides) • LHP portion, legible, signed and dated • Parent portion, legible, signed and dated 2. PROPERLY LABELED • Pharmacy label (prescription) • original container w/student name (OTC) • Epi-pens and Inhalers- Pharmacy label on box, device or holder labeled w/name

Intake of Medication (cont.) • COUNT MEDICATION w/Parent • On clean surface/tray • Parent to split pills if indicated, before counting • Both parent and staff sign and date log • If parent/adult unavailable for count: two staff count/date/sign; contact parent to report # of pills received • DOCUMENT AMOUNT OF MEDICATION Remaining, Received and Total 5. IMMEDIATELY STORE IN SECURE LOCATION Contact your school nurse with questions/concerns or for any requests for non-oral medications/treatments If a medication is expired, contact your school nurse

  13. Administering Medication(page 13) • Call student to the med area • Student to have cup of water • Identify student • Check container against the LHP written orders • Check medication log to be sure medication has not been given

Medication Administration cont. • Pour medication into cap, then student’s hand • Watch student take medication • Record time and your initials in the date box immediately after giving the medication • Use correct documentation No white out, Use only blue/black ink No empty date boxes for daily meds • Return medication to secured/locked storage area

  15. Medication Don’t’s(page 13 #9) • DO NOT prepare any medication ahead of time* • DO NOT give medication that someone else has prepared* CONTACT YOUR NURSE WITH ANY QUESTIONS OR CONCERNS *except when preparing medication for a field trip

  16. REMINDER: Private Duty Nurses and Contracted Nurses (i.e. Maxim) can only give medication to the specific student they are providing care for.

Students Carrying Medication (page 14) Requirements for students self carrying oral meds: The student must be able to self administer without any assistance • One day’s dosage only • Note from parent with: • Date(s) the student will be carrying the medication • The name of the medication • Parent signature and parent contact information • LHP name and contact information Any student may lose the right to carry medication if they are not using it responsibly.

Field Trips-General(page 16) • Personnel who are trained in the administration of medication must accompany students that need medications given on a field trip. • Nurse needs 72 hours notice to set up a training • Office staff will prepare medications for the trip (this is the exception to never prepare meds ahead) • Medications will be kept “secure” at all times • The student that self carries is responsible for their own medications • Contact nurse with questions/concerns

  23. Medication Administration Time (page 17) • The dose must be given within 30 minutes before or 30 minutes after the time designated on the order. • If LHP orders a lunchtime dose. The dose must be given within 30 minutes before or one hour after the students lunchtime.

  25. Medication Error-NOW WHAT! (page 19) • Notify principal, parents and school nurse. • Document the error in the comment section of the log • Remember this is not the end of the world, we learn from our experiences
