Policy Context Pharmacy in the Future - Implementing the NHS plan All PCTs to have access to a range of medicines management services by 2004 Effective Medicines Management: Re-shape care around the patient Commission for Health Improvement (CHI) Reduce risk ‘An Organisation with a Memory’ & National Patient Safety Agency (NPSA) Improve health ‘Saving lives: Our Healthier Nation’ (OHN) Make better use of skills and knowledge of staff local contracts such as PMS and LPS

What are Medicines Management Services (MMS)? • Systems for designing, implementing and delivering accessible, appropriate and cost-effective pharmaceutical care to patients based on need • Can include all aspects of the therapeutic use of medicines from organisation to individual patient level

  5. Medicines Management can be grouped into five broad types • Clinical • Systems and processes • Health of the public • Interface • Patients and their medicines

  6. Why do organisations need MMS? Medicines are important to the public’s health Medicines may not be used as intended Uncertainty about outcome of interventions There is variation in quality of service delivery Systems break down at the interface

Evidence of the need for MMS Half of all patients with chronic conditions do not use their medicines as intended Medication problems are implicated in 5-17% of hospital admissions Medication errors have been estimated to cost the NHS £500 million a year in additional days spent in hospital People who could benefit from medicines are not receiving them

  8. Questions patients may ask about their medicines 1. Do I need a medicine? 2. How do I find out about different treatments 8. Do I keep taking my medicines or when can I stop? 7. How can I make sure my medicines suit me? 3. Who decides which medicines I get? 6. How do I get my medicines? 4. How am I involved? 5. How do I find out about my medicines?

MEDICINES MANAGEMENT IN ACTION:Common Medicines Management Services • Clinical medication review • Services to residential and nursing homes • Medication monitoring and support • Patient & carer information and education • Review of repeat prescribing systems in GP surgeries • Pharmacist or nurse-led clinics in primary care • Therapeutic priorities • Older people, mental health, CHD, vulnerable groups • NICE guidance • Community pharmaceutical services • Minor ailment referal systems
