Phase 1 – 52 organizations participated All hospitals (including acute mental health and regional cancer programs 9 Community Care Access Centres 6 Community Health Centres The Group Health Association Phase 2 - 400 community health organizations addictions, children’s treatment, community support service providers, independent health facilities, long term care homes, mental health, public health 200 community pharmacists 43 nurse practitioners 28 family health teams 1,100 physicians Volunteers Chronic disease and patient management programs The most comprehensive e-Health ICT Blueprint in Ontario, covering the continuum of healthcare service providers

Strategic Framework • Improve consumer access to health information to support informed decision making • Improve communication among service providers • Improve care delivery • Improve service delivery management • Enable information integration and communication among stakeholders and sectors • Commit to research, education and knowledge transfer • Maximize available funding • Ensure a planned and coordinated approach to eHealth and ICT in Northern Ontario Northern Ontario ICT Vision Information and Communication technology supports the processes of quality healthcare provision, access to health information, improvement of consumer outcomes, and the most effective use of available resources across Northern Ontario, through collaboration and sharing information amongst providers and with consumers Northern Ontario e-Health ICT Goals

Northern Ontario e-Health ICT Strategies • Establish the Electronic Record within individual organizations • Establish capability to share information among electronic records to facilitate information integration • Use technologies to promote service provider communication • Use technologies to support consumers to achieve improved health outcomes • Build on existing provincial initiatives – WTIS/EMPI, OTN, PACS, OLIS • Develop system-wide decision support capability, facilitating evidence-based decisions • Support work effectiveness • Support research and education • Develop technical and support infrastructure and utilize new technologies to match requirements • Maximize available resources

Governance Principles • Includes all stakeholders from across the continuum of care; • Supports expeditious planning and implementation of the Blueprint directions; • Has a structure and process for communication and input from the stakeholders; • Advocates for and supports a change management and adoption/ uptake program to support the implementation of the e-Health ICT Blueprint; • Leverages current strengths, structures and experience in Northern Ontario; • Promotes an integrated approach in Northern Ontario, while respecting the unique circumstances of communities and service providers; • Supports LHIN accountability agreement for e-Health; and • Is proactive in supporting adoption of innovative technologies that are of benefit to health information management in Northern Ontario
