  3. The Marketplace is NOT the same as Medicaid! Marketplace  Private insurance companies • State Medicaid (NJ FamilyCare) How are they related: • Eligibility and how much you save are INCOME-BASED • The Marketplace online application will send your information to NJFC if you qualify for Medicaid Marketplace vs. Medicaid (NJ FamilyCare) www.healthcare.gov www.njfamilycare.org

Get Help LOCALLY Local Help Get help applying in person There are 20 federally qualified health centers throughout the state of New Jersey, with a total of 113 sites All 20 are functioning as CACs Two (2) are in Middlesex County • Jewish Renaissance Medical Center, Perth Amboy, NJ • Eric B. Chandler Health Center, New Brunswick, NJ

Outreach and Enrollment Our FQHCs are focusing on In-reach and Outreach In-reach:Enrollment of existing patients who are currently uninsured and are eligible for enrollment in Medicaid expansion or the Marketplace. Outreach:Identify and enroll new patients and community members who reside in the communities in which our centers and satellites are located.

Requirements for Assisting people to Enroll MUST BE CERTIFIEDby Centers for Medicare and Medicaid Services (CMS) • They must complete and pass a web-based training course with a passing score of 80 • You can be confident that you’re getting the best service at our state’s federally qualified health centers Health Center staff must be certified to be CACs

  11. Employer Coverage Tool • Only if employed, and/or are eligible for health insurance from your or another person’s job • Complete one tool for each employer that offers health coverage that you’re eligible for Job Health Insurance
