6. Assessment of Risk to the Child’s Safety At the Safety Assessment, the threat of harm must be serious and imminent

10. Completing a Safety Assessment Minimum Contact Requirements • Face-to-Face contact with • the alleged child victim(s). • At least one of the caretakers • must be interviewed face-to-face.

Timeframes for Safety Assessment • Completed within four (4) working days from the receipt of the report. • If initial attempt to make face-to-face contact is unsuccessful: an additional attempt within the first four (4) working days of the assessment/investigation is required. • After face-to-face contact is made, the Safety Assessment tool must be completed the next working day.

Safety Plan Specific and concrete strategy for: • controlling threats of serious harm to a child(ren) or • supplementing protective capacities • implemented immediately when a family’s protective capacities are not sufficient to manage immediate safety threats for at least one child in the family.

Safety Plan Guidelines • Safety Plans must be continuously re-evaluated and modified, whenever necessary. • In-home safety plans – • weekly home visits by PCSA • Out-of-home safety plans – • weekly contact by PCSA with the children or persons responsible for an action step, either by telephone or face-to-face • and have face-to-face contact with each child involved every other week

15. Discontinuing a Safety Plan • When a threat of serious harm no longer exists or • When control of the threat within the family is probable and can be maintained without PCSA safety focused intervention or active safety plan monitoring. • All parties of the Safety Plan need to be notified in writing within one working day of the Safety Plan being discontinued. • CPS cases cannot be terminated, outside a court order, when a Safety Plan is active.

25. Reunification Considerations • Are safety threats active that require external control? • If so what controls are necessary? • Have threats been resolved, altered or reduced to a level whereby control within the family is probable?

Reunification Assessment • May be completed at anytime PCSA deems it appropriate. • Must be completed: • At Safety Re-Assessment when the safety response is Modify or Discontinue and the child has been placed out of the home for 30 days or more. • Within 30 days prior to any court hearing where the agency is anticipating the child will be returned home.

Specialized Assessment / Investigations • Alleged perpetrator meets one or more of the following criteria: • Is responsible for the care of a child in Out-of-Home Care Setting • Is responsible for the care of a child in Out-of-Home Care • Has access to the child by virtue his/her employment or affiliation with an institution

28. Ongoing Assessment & Investigation • If Intra-familial child abuse and/or neglect is alleged in a case which is opened for ongoing agency services… A Safety Assessment and Ongoing Assessment/Investigation must be completed.
