  2. Background Sam is 14 years old and in 8th grade. He does not finish work due to off-task behavior. He rushes through his assignments making errors and overlooking information. Off task behaviors include: reading, doodling, daydreaming and talking to friends. Off task behaviors occur during independent work times and not during group work. He earns good grades and participates during group work. At home, he is full of energy and plays sports.

Action Plan for Teacher Integrate group work after direct instruction Use learning centers Provide choices for hw assignments Provide choice to work alone or in groups Give him incentives to work independently ex. Read or draw once independent work is done ex. Provide independent work log and offer rewards. Utilize think-pair-share activities

  8. Action Plan for Parents Use sports/playtime as incentives for finishing hw ex. Sam can play for 30 min. after school, but then has to work on hw and finish before he can go back out and play Be sure to spend time talking with Sam about what he is learning in school. Encourage independent work at home using the independent work log

  9. Action Plan for Student Participate during direct instruction Take time and double check independent work Set goals for independent work time ex. Keep a log for time spent on independent work Set time goals for certain assignments ex. Spend 20 minutes on this assignment
