  1. NEW HIRE PROCESS 1. DRIVER QUESTIONNAIRE: THIS IS FREE!!!!!! * If they are required to drive, send a copy of valid driver’s license and Driver Questionnaire to Daisy Pearson in Safety. daisy.pearson@pathfinderinc.org 2. CRIMINAL BACKGROUND CHECK $22.00 * Supervisors from the facilities will send over a request form for a Criminal Background Check to TANYA EDWARDS. This is done BEFORE the Drug Screen. NO P.O BOX FOR ADDRESS Academy, PS1,and PS2 will fill out a special criminal background check this form is in your packet, IT’S FREE!!!! NOTE: CRIMINAL BACKGROUNDS ARE RAN EVERY 5 YEARS ON ALL EMPLOYEES. 3. DRUG SCREENS ONSITE $35.00 OFFSITE $45.00 * Drug Screens are done Monday through Wednesday, CALL TANYA EDWARDS. The results will be back in 24 to 48 hours. Tell the new hire not to have anything in their mouth 10 minutes before testing and to bring their driver’s license. * If the drug test is GOOD, set up a new hire appointment, CALL CARRIE WOMBLE. RANDOM DRUG SCREENS Three employees are randomly selected on Board Meeting Days. Tanya will notify the supervisor and let them know who has been chosen for a random drug screen. The employee has 1 hour from the time they are notified to come to the administration building. If it is an outlying facility, the supervisor will send the employee to where they normally send them for drug screens. THE EMPLOYEE MUST BE TESTED ON THE DAY THEY ARE NOTIFIED, NO EXCEPTIONS! 4. I-9’SEmployee completes section 1, Supervisor completes section 2 and certification. Make sure you have a double sided form. Supervisor will send I-9 to TANYA EDWARDS within 3 days of hire date. Example: If they are hired on August 6th they are required to be entered in the system by August 8th, NO EXCEPTIONS! FAX # 501-533-6359

NEW HIRE New hire appointments are done Monday through Thursday, CALL CARRIE WOMBLE Supervisor will need to send over a Change of Status (COS), Application and Job Description before new hire appointment. Things they will need before appointment. * Current Driver’s License or ID card * Social Security card or Confirmation of Card * Proof of Education * 10 Years of Addresses * TB test * Car insurance card For outlying areas complete a New Hire Packet and submit to Carrie Womble with everything listed above. ADULT AND CHILD ABUSE FORMS Completed on all employees. Academy, Preschool 1, and Preschool 2 will complete a Child Maltreatment form. Mental Health will complete a Child Maltreatment and Child Abuse. NOTE: ADULT & CHILD ABUSE FORMS ARE RAN ON ALL EMPLOYEES EVERY 2 YEARS. NO P.O BOX FOR ADDRESS FINGER PRINTS If the person has lived out of the State of Arkansas in the last 6 years, we must do finger prints on them. ICF/DD’S HAVE TO FINGER PRINT ALL EMPLOYEES Academy, PS1, and PS2 HAVE FBI FINGER PRINT FORMS THAT MUST BE COMPLETED AND SENT IN WITH EMPLOYEES NEW HIRE PACKET.
