(2014 - $730, 2015 - $1,460) • Reinsurance Fees – Health insurance issuers and self-funded group health plans must pay fees to a transitional reinsurance program for the first three years of health insurance exchange operation (2014-2016). $5.25/month ($63/yr.) x average number of covered lives. ($45,990 / Yr.) • Excise Tax (Cadillac Tax) – 40% excise tax on employers for the value of health insurance benefits exceeding established thresholds.
