Smoke 2. engage in illegal acts using the dormitory internet 3. keep visitors staying overnight without permission or allow the opposite sex to enter dormitory rooms 4. cook inside the dormitory rooms or install electrical appliances ----unauthorized by the university 5. Solicit : introduce business people into the dormitory to trade goods

  24. Operating Hours of Clinic • Mon-Fri:8:30am-21:30pm (schooldays,Spring & Summer vacation) • Doctor’s clinic:Mon to Fri: 2:00pm-4:30pm • (Spring&Summer break -- Mon、Wed、Fri. only)

  26. Student Group Insurance During enrollment, you have to pay a student safety insurance. It is required that all student should have student safety insurance. Definition: The insurance company will pay the benefit when students suffer accidents or diseases when medical treatments are needed. The insurance company will also make reimbursements due to accidents which caused student’s disability or death. Range of insurance: Out-patient clinic only for accidents. In-patient treatment. Students suffer accident, diseases in foreign countries.

Student Group Insurance Document to be submitted when applying for student insurance: 1、Application form.(Contact NTUST health services division,Gymnasium 1st Floor) 2、Medical certification. 3、Hospital receipts (original or certified true copy.) 4、Photocopy of bank passbook and ARC 5、It could be applied within 2years afteraccident or illness happened.

Student Group Insurance Document to be submitted when applying for student insurance: 6、Student ID card copy and Bankbook copy 7、Out-patient cases,Hospitalized, Surgery, Illness that requires observation for at least 6 hours, meets the requirement criteria for group insurance payment. 8、Within 2 years can apply. 9、If accident happened overseas, you will need to provide local medical certification and hospital receipts in English.

Health Screening and Examination Health Examination for All New Students Time :Sept 20th-21th,2014 (8~12am 1~4pm) Location :2nd floor,Gym Fee :About 550NTD(cash) Note : Be sure to come right on time with one passport sized photo. Three days before the health check, please make sure you have enough rest and drink more water. On the day of the health check, please do not take food and water 1 hour prior to the examination. On the day of the health check, please do not wear clothes with metal and shiny beads. Please do not wear any makeup, nor bring valuable stuffs. Pregnant ladies or if you suspect that you may be pregnant, please do not take X-ray scans.

EBOLA • Returning to Taiwan from epidemic areas, please practice in the 21 daysself-management of health. • Wash your hands often with soap and water. • If you get cough, please put on a mask. • If you feel ill, please call the health center 2737-6126 or toll-free hotline 1922(or 0800-001922).
