  4. Incident Information • Date November 23, 2006 • 257 Elm Street Atlanta • Single family converted to duplex • Single story on partial basement ordinary construction • 36’ X 32’ = 1152 sq ft • Fire flow requirement 480 GPM • Vacant and boarded up

  6. Incident Timeline • 2003 - Dispatch • 2005 – E16, E1, Tk16 on scene “heavy smoke pushing, no visible fire” • 2007 – E7, Tk1, B3, Sq 4 on scene • 2008 – Flash occurs – B3 orders “defensive” • 2009 – Report of persons in building • 2010 – Sq 4 forces door “Side C”, TIC in use

  7. Incident Timeline • 2011 – TK1 request PAR, crews still inside • 2011 – E16 acknowledges PAR, “whole crew accounted for” • 2011 – E7 reports FF down, Sq4 locate and remove • 2016 – Grady EMS on scene • 2016 – Fire under control • 2017 – FF 10-76 Grady via Grady EMS

  8. Significant Times • E16 on scene to flash – 5 minutes • FF down to transport – 6 minutes • Alarm to fire out – 13 minutes • On the fire ground things can and do go bad quickly !!!!!

  9. Major Influencing Factors • Converted construction • Open interior spaces with no sheetrock finishing • “Lighter pine” wood used for construction, sap is almost a flammable liquid • No immediate ventilation

  10. Major Influencing Factors • Crew experience • No confirmation of defensive operations • Acting Officer acknowledged PAR • No walk-around performed • Many units on scene quickly, lack of direction, IC overwhelmed, many FFs at front door

  11. Lessons Reinforced • Training • Assure personnel are properly trained for their position • Assure personnel are trained with TIC and equipment is used • Assure personnel understand Fire Behavior and critical event forecasting

  12. Lessons Reinforced • Training • Always stay low • Understand disorientation • Vent early • Cool superheated air with water • Frequent gear inspections • No “under armor”

  13. Lessons Reinforced • Training • Enforce, practice and use PAR accountability • Enforce, practice and use RIT correctly • ( 5 personnel used for extrication of FF) • Do not allow crews at front door to pull hose line out from interior when changing operations • Maintain crew integrity
