2. Training • Training arranged by Center for Health and Behavioral Training – Univ. of Rochester www.chbt.org • Orientation – 1 hr • Intervention – ½ day • Booster – 2 hrs

3. Interventional Materials • Posters in waiting area & exam rooms • Brochures (English & Spanish) • Placed in nurses’ stations • Informational Flyers (English & Spanish) • Placed in exam rooms

4. Patient presents to clinic for appointment: *Nurse briefly discusses PfH & offers brochure *Patient reads brochure while waiting for provider During medical visit: *Provider assess patient’s risk of transmitting HIV/STIs *If at-risk, merge PfH template Click Templates PfH Workflow Under Choose Templates, Check Generic and under Category choose Ryan Templates Or My Favorites Select PfH-Safer Sex Counseling and click Merge Template Provider performs a 3-5 min intervention *Brochure is reviewed with patient *Consequence/Advantage-framed questions asked *Informational flyers provided as needed *Goals discussed, referrals made if needed *Counseling is documented in the chart by selecting drop-down menu under Patient Discussion At each follow-up visit, provider *Inquires about progress on goals *Reinforces healthful behavior *Merges in PfH template & re-establishes Goals for next visit

PfH Challenges/Moving Forward • Incorporating intervention into general medicine clinic • Confidentiality • Practitioner may not perform intervention on all at-risk patients (nurse merge template?) • Goal-setting • Expansion to non-clinical staff (modification) • Ensuring follow-up conversation with clinician • Posters/written materials need improvement
