    5. 4 Step Plan for Improvement IMPROVE – Rancho Bernardo Pilot 1. Updated Nursing Competency/ Standardized Rooming 2. Clinical Guidelines – Blood Pressure Management in Patients with Diabetes 3. Patient Educational Handout 4. Monthly Physician Reports Since we last met, we have begun to do things at the Rancho Bernardo pilot site to improve control. We updated the Nursing Competency materials and implemented training for all nursing staff at the Rancho Bemardo pilot site. The Physicians developed and approved clinical practice guidelines for BP control in patient with diabetes We created and are using a patient handout in the offices. We created and are distributing monthly physician reports. Since we last met, we have begun to do things at the Rancho Bernardo pilot site to improve control.
