Thailand: Situation analysis of TB and HIV • High TB burden and high HIV burden • New HIV infection : • 150,000 in 1991 • 10,800 in 2010 • TB found in 1 third of symptomatic HIV & AIDS • Population: 70M • Universal coverage scheme 75%

Thailand: Situation analysis of TB and HIV • In 2010, 90% of TB cases were tested for HIV co-infection, out of which 16% were found to be HIV-positive. • Out of the co-infected cases, 71% are receiving CPT, while 53% are receiving ART. • There was a TB/HIV guideline since 2008.

Number of HIV-positive TB patients enrolled on co-trimoxazole preventive therapy (CPT) and antiretroviral therapy (ART), 2004-2011
Proportion of TB patients with HIV infection received ART (Thai & Non-Thai) : 2006 - 2013 ART
TB Cases Detected among PLWHA (Thai & Non-Thai) : 2006 - 2013
Treatment Outcome of New M+ by HIV status (Thai & Non-Thai) : 2012
Specific interventions • BTB & BATS coordination: • strategy management committee (4+ meetings/year), • Involvement in the new HIV guidelines development • M&E: Implement the TB/HIV report form • TB screening among the HIV infected • HIV screening among the TB patients • ART among the TB/HIV • Conduct studies to identify the cause of death among TB patients including TB/HIV
Specific interventions • Information use for policy and program advocacy : AIDS Zero Portal • TB/HIV mortality • Screening for HIV among TB casesand for TB among HIV infected • ART in TB/HIV cases
Challenges and lessons learned • Coordination body in all levels • High mortality in TB/HIV: • Delay diagnosis of HIV and start treatment • Notification in private sector • Selection of indicators for policy and program level (AIDS Zero Portal)
Next steps • Establish formal coordination body in different levels • Setup TB expert committees in regional levels that will closely work with HIV expert committee • TB/HIV will be monitored through AIDS Zero Portal in 2014 • Study on cause of death in areas with high TB/HIV mortality
ART Target WHO recommendation : Any CD4 count of all TB/HIV patients should be consider ART treatment. Thailand : CD4 < 350 , ( 70 % of TB/HIV patients )
Antiretroviral treatment uptake in patients with HIV-associated TB
TB-IRIS ❖ Incidence in HAART era: ~30% ❖ Risk: miliary/disseminated TB baseline CD4<50 HAART started within the first 2 mo. of TB treatment
HAART in TB/HIV • high pill burden • overlapping ADR • TB-IRIS • long-term metabolic complication • develop other OIs or conditions associated mortality Delayed Early
