  2. Flujo sanguíneo uterino • El flujo sanguíneo uterino carece de autorregulación. • Depende de la presión arterial y del gasto cardiaco materno. • Como consecuencia de esto los factores que alteran el flujo uterino afectan de forma adversa al aporte sanguíneo fetal.

  3. EL FLUJO SANGUÍNEO FETAL Flujo sanguíneo uterino= Presión arterial uterina- Presión venosa uterina Resistencia vascular uterina • El flujo sanguíneo uterino disminuye durante los periodos de hipotensión materna, que pueden producirse como consecuencia de hipovolemia, hemorragia, compresión aorto-cava y bloqueo simpático.

Que disminuye el flujo uterino • Los anestésicos pueden alterar drásticamente el flujo sanguíneo uterino. • El bloqueo simpático ALR puede provocar hipotensión materna. • La administración de fluidos previa no evita por completo la hipotensión materna, pero aumenta el gasto cardiaco materno y por tanto puede preservar el flujo

Vasoconstrictores y flujo sanguíneo uterino • El uso de vasoconstrictores del tipo efedrina o fenilefrina, conserva el flujo uterino durante la ALR. • Se utilizan como prevención o tratamiento de la hipotensión secundaria a anestesia espinal en las cesáreas. • Existen numerosos estudios con diferentes dosis o regímenes de infusión de los mismos.

  6. Farmacología Fenilefrina • Es un -agonista puro, ampliamente utilizado en el tratamiento y prevencion de la hipotensión tras el bloqueo espinal en la gestante. • Mantiene las resistencias vasculares periféricas, con lo que utilizada a dosis correctas mantiene la presión arterial materna. • Evitando: nauseas, vómitos y mareos.

  7. Fenilefrina efectos secundarios maternos • Puede provocar en las madres bradicardia refleja que podría disminuir el gasto cardiaco materno

Efectos Fenilefrina sobre el feto • Un metaanálisis realizado por Reynolds y cols. mostro que la anestesia espinal para las cesáreas se asocia con una disminución del pH del cordón, si se la compara con la anestesia epidural o la general. • Estos autores sugieren que el uso de dosis mayores en las pacientes que recibieron efedrina en las pacientes con anestesia espinal fue lo que contribuyo en gran medida a estos resultados.

Habib, MBBCh, MSc, MHS, FRCA • This review highlights the effects of phenylephrine compared with ephedrine on maternal hemodynamics (arterial blood pressure, heart rate, and cardiac output), and occurrence of intraoperative nausea and vomiting. • The impact of the administration of phenylephrine as a bolus for the treatment of established hypotension compared with its administration as a prophylactic infusion is discussed. • This article also reviews the impact of phenylephrine compared with ephedrine on uteroplacental perfusion, and fetal outcomes such as neonatal acid-base status and Apgar scores. Habib, MBBCh, MSc, MHS, FRCA • Bothephedrine and phenylephrine are effective in managing spinal anesthesia-induced hypotension. • Phenylephrine may be associated with a lower incidence of IONV, and higher umbilical artery pH and base excess compared with ephedrine. • Administration of phenylephrine as a prophylactic infusionis more effective in reducing the incidence of hypotensionand IONV compared with bolus administration. • However, phenylephrineuse is associated with a decrease in maternal cardiac output.

Conclusions Phenylephrine and ephedrine are acceptable choices to combat maternal hypotension related to spinal anesthesia in elective Cesarean section. Complications of intra¡operative nausea and vomiting, tachycardia and bradycardia should be considered when choosing a vasopressor, suggesting phenylephrine may be more appropriate when considering maternal well-being.

Reanim. 2011;58:412-416 Conclusiones La efedrina y fenilefrina a las dosis estudiadas, han demostrado tener similar potencia para prevenir la hipotensión arterial durante la anestesia subaracnoidea. El perfil de efectos secundarios mostro una mayor incidencia de hipertensión arterial y bradicardia en el grupo de la fenilefrina. No hubo diferencias en los efectos neonatales.

Conclusiones de los autores Aunque intervenciones como administracioón de coloides, efedrina, fenilefrina o compresión de las piernas pueden reducir la incidencia de hipotensión, ninguna ha demostrado que evite la necesidad de tratar la hipotensión materna durante la anestesia espinal para la cesárea. Debido al relativamente escaso número de mujeres estudiadas no se pueden establecer conclusiones con respecto a efectos adversos poco frecuentes.

  16. Experiencia personal • Nuestra experiencia se esta haciendo en pacientes sometidas a cirugía fetal intraútero. • Hemos tenido que cambiar el protocolo con fenilefrina en infusión continua debido a la bradicardia materna y a la bradicardia fetal. • Evidenciada mediante ecografía continua fetal en precordial. • El cambio a una infusión de efedrina ha subsanado estos problemas que tuvimos al principio.
