  3. Who We Are… Born, in 2001, from a convening intent to more effectively address common problem of uncompensated care Public/private partnership of county governments, community leaders and providers with common mission of improving communities’ physical and fiscal health

  4. The initial vision….. Labs/Diagnostics Medications Social Services Primary Care Specialty Care PATIENT Hospital Care A and D Services MH Care • HCCG/CHW 2004 … will save money.

Our Initial “Product”: • Enrollment based program to serve… • Uninsured adults 19-64 years old with incomes under 200% FPL and either hypertension, heart disease, diabetes or depression • because chronic unmanaged conditions consume a disproportionate share of costs and are amenable to outpatient care • Three components: • Access to continuum of primary and specialty care, labs/diagnostics, life-sustaining pharmaceuticals and hospital services • Evidence based health promotion and disease management • Holistic care management (including transport PRN)

  10. ER and hospitalization rates 27% and 13% less, respectively, than the control group of singly diagnosed chronically ill individuals. That translates to $500,000+ in annual savings to regional health system per thousand members, AND… Evaluation Shows…

April 2001 through February 2004… • $1.9M in local cash reprogramming for outpatient/pharmaceutical uninsured care • Beginning 1 new clinic, serving an additional 225 patients currently • Coordination of more than $9.5M in new care across 100+ providers • Primary and specialty (including behavioral health, dental and eye) • Public health • Labs and diagnostics • Hospital • Pharmaceuticals, supplies and DME • $800K+ in pharmaceutical assistance medications for indigent patients • $1,381,549 in new federal and national philanthropic investment to Georgia • $175K in new state philanthropic investment • $1M+ recovered reimbursement for the region’s healthcare system

Demonstration that health/healthcare, partnership and care coordination are economic development engines in communities through more than $2.3M cash economic development impact since 10/01 in 7 rural /suburban counties • Broad and growing regional partnership committed to synergistically improve health

Has brought national recognition to Georgia • Robert Wood Johnson Foundation • National Association of Counties • National Conference of State Legislatures • U.S. Community Health Leadership Network • Stimulated new possibilities • Broad support of CHW as Medicaid care management vendor • Organizational home of cancer RPE planning • Invitation to partner to develop IT “super-system” for national communities committed to access • Request to assist brokering national partnership to support the power of coordinated and engaged communities to improve health

  15. Sustainability… Application of core competencies to covered population Expand IT to serve other markets Demonstrated value to partners and investors Use developed ability to form and sustain “health partnership in action” to leverage new investments

  16. Lessons Learned: • Local, entrepreneurial leadership and tangible commitment to building a system not a project is “make-or-break” in true network development. (Board and Staff) • Networks are logical vehicles to synergistically address multiple health challenges. • Positively impacting access and health status is about systematicleverage.
