Safety Learning: Could This Happen in Your OC?Anadarko, Dropped Rabbit RI…August 22, 2008 Brief Account of Incident: A drilling crew was drifting 2 7/8” drill pipe with a rabbit while running in the hole. On the 90th joint the rabbit was placed in the pipe which was then rolled onto the hydraulic catwalk. The drill pipe was raised to the rigfloor and the elevators were attached. The pipe was picked up and the joint slid on the catwalk towards the rig floor. Once the pipe was raised off the catwalk the derrickman identified the rabbit had not exited the pipe. The pipe was approximately 4’ off the catwalk when the rabbit dislodged unexpectedly striking the catwalk. The rabbit glanced off the catwalk and struck the derrickman on the right forearm with a small cut as a result. Operations were suspended immediately to report the incident. A decision was made to transport the injured person to the local hospital for medical treatment. The injury required five sutures and the injured person was released back to work. • Incident Findings: • Drifting pipe with a rabbit while running in the hole is very repetitive and requires concentration to maintain a constant work rhythm. • Debris in drill pipe allowed rabbit to become stuck in pipe. • Operator continued to raise pipe off catwalk increasing the distance for the rabbit to drop. • Although an attempt was made, the derrickman failed to alert the floorhand to stop picking up the pipe before the rabbit dislodged unexpectedly. • Inadequate buffer zone maintained by derrickman. 2 1 Key Learnings: Maintain safe buffer zone on ground until pipe clears V-door As the pipe becomes vertical slowly raise pipe to ensure rabbit is not stuck and exits pipe Stop operations if rabbit becomes stuck and clear area Consider drifting pipe on rack 4 3 5 Approximately 15” and 2.5 lbs.
