Student Health Services • Provides primary care to all enrolled students • OU insurance is not required to utilize services • Primary Medical care • Pharmacy • Physical Therapy • Gynecology Services • X-Ray • Allergy Injections & Immunizations • Minor Surgical procedures • Psychiatry (limited) • Laboratory

  4. Health History Form • Form must be completed and returned to Hudson Health Center • Actual dates of immunizations are needed • Add notes from your physician if needed • Attach a copy (front & back) of your current insurance card

  5. Immunizations • None are required • Meningitis and Hepatitis B are recommended • Immunizations are available at Hudson by appointment (Hep A & B, Polio, Meningitis, MMR, Rabies, Typhoid, Yellow Fever, Td, Tdap, Gardasil, and Influenza)

Fees and Charges • Beginning September 2008 medical services will be billed to your insurance company. • Also beginning in September, each quarter your student bill will include the $40 WellBeing Plan. This fee will allow us to add more hours and additional providers & counselors. Paying this fee will also waive your co-pay and/or co-insurance payments at Student Health Service. • You will not need cash at Hudson. Charges for pharmacy, any co-pays or co-insurance (if you have waived the health fee) will be charged to your student account. SHS will bill your insurance for provider visits, lab, and x-ray.

Counseling and Psychological Services • Educational Counseling • Study habits, panic about exams, discouragement about academic progress • Career Counseling • Explore personal needs, interests and aptitudes. • Personal Adjustment Counseling • Personal worries, depression, lack of confidence, fears, unsatisfactory relationships.

Accident and Sickness Insurance Policy • All students are required to have a health insurance plan while they are enrolled in the University. • All students are automatically billed for a health insurance plan offered by Ohio University. • Domestic students who are covered by an insurance plan may waive the University-offered plan; you will do this electronically when you receive your electronic fee bill. • You must waive the insurance by the deadline waiver date indicated for that quarter. • The WellBeing Plan is different from the Student Insurance charge. If you have private insurance you may waive the insurance plan, but can opt to participate in the WellBeing Plan.

Eligibility • Domestic students taking 7 or more credit hours on the Athens Campus • International students taking 1 or more credit hours on the Athens Campus • Special category options available for special circumstances (i.e. grad students, co-ops, internships, visiting scholars, J1 Visa etc.)

Spec. (8/1-31/09) NA Bursar must receive e-bill or hard waivers prior to deadline waiver dates above.

Student Health Center Referrals Students must always go to Student Health Service for care if: • Student Health Service is open M,T,W,F 8am-4:30pm TH 9am-4:30pm (Evening/Weekend hours coming soon) • It’s NOT an emergency (Not sure if its an emergency? Call My Nurse Line at (800-883-2951) • The student is within 50 miles of Student Health Service

More About Referrals • Referrals are made for a specific illness or injury (not provider) • When seen outside Hudson in a Medical Emergency, students are to return to Student Health Service for follow-up care and further referral if needed • Referrals are good for that policy year only • If you are seen at Hudson Health Center and receive a referral from one of the providers, you must go to the insurance office (Rm. 233) at Hudson and have an the insurance referral form will be done on line.

What to do when you present for medical care outside of Hudson Health Center • Choose a network provider for maximum reimbursement (United Health Care) • Show your insurance card to the provider and pay your co-payment • Fill out a student medical insurance referral form completely. Indicate the reason for not going to Hudson. Claim forms are available at Hudson Health or on the insurance website at www.uhcsr.com/college/viewbrochures.aspx (Down arrow to Ohio University under select your school) this will bring up options for the brochures, forms, and insurance cards.

WHAT TO DO WHEN YOU PRESENT FOR MEDICAL CARE OUTSIDE OF HUDSON HEALTH CENTER CONTINUED… • Send the completed claim form to the insurance company. • You may choose any provider however be aware out of network providers are only covered at 50% as apposed to in network at 100%; also when you do choose network provider you will receive network pricing which may reduce your out of pocket expense. • The above process works the same for dependents. Be sure to complete the dependent box on claim form.

  21. Pharmacy Benefits • Sickness & Injury prescriptions filled outside of Hudson Health have co-pays: Tier I ($10) or Tier II ($25) Show your insurance card • Prescriptions filled at Hudson Health are 100% reimbursement
