WHAT IS THE PURPOSE of a PUBLIC HEALTH/DIABETES TEAM IN the COMMUNITY? • Promoting Healthy Practices: Monthly Health Screens • Preventing Disease and Injury: Providing Mass Immunization Clinics • Advancing the Health of the Community: Educating Community with flyers, posters, handouts, & presentations • The Public Health Department received 100% on the CA Community Health Services Audit Spring 2009, and were complemented on program organization and management. • Best Practices Award received in Spring 2010, for outstanding GPRA improvements in over 10 health measurement areas.

  4. WHAT DOES OUTREACH DO FOR THE COMMUNITY? Lots! • IN-HOUSE RETINAL SCANS • HEALTH SCREENINGS MONTHLY with COMMODITIES DISTRIBUTION • CAR SEAT CLASSES for FAMILIES • DIABETES and PRE-DIABETES EDUCATION CLASSES • DIABETES CASE MANAGEMENT • HOME VISIT PROGRAM • WELLNESS GARDEN • EMERGENCY PREPAREDNESS • FITNESS AND WELLNESS CENTER with PERSONAL TRAINER • AFTER SCHOOL FITNESS PROGRAM • ELDER DINNER DANCES ,QUARTERLY

HOME VISIT PROGRAM • A home visit may include weekly blood pressure and blood sugar checks, community resource assistance, Diabetes education, nutrition counseling, transport to a medical appointment, or medication pill-box set-up. • We see 30 patients regularly each week by Public Health Team, the majority have chronic disease and/or Diabetes.

WELLNESS GARDEN • A Medicine Wheel Garden was planted near Community Wellness Center. • Herbs, fruits, and vegetables were plentiful this summer. • We planted the garden to symbolize the Native Medicine Wheel—offering balance to the community. • Many elders received garden veggies over the summer it was not much but-- greatly appreciated by all. The four spheres represent: • Mental • Spiritual • Physical • Emotional Health

  7. FITNESS AND WELLNESS CENTER • Johnny Gonzales operates the gym, and serves as the Fitness Trainer. He provides 1:1 fitness and nutritional education to Native American patients. • Approximately 35 people utilize the gym on a regular basis, from ages 8- 84. • The California Indian Health Service, Diabetes Director complemented Lake County Tribal Health on our Community Fitness and Wellness Initiative, LCTHC Public Health Director spoke at annual meeting regarding successes of the program.

RETINAL SCANS • Retinal Scans performed by Public Health staff, photo forwarded to a UC Davis physician for interpretation. • When we began retinal program, February 2009, only 12% of Diabetic population had received eye screens. We are now at 67%, surpassing our previous goal each year, many have received cataract/laser surgery, reducing the need for eye glasses. • Diabetic patients have been our target, over half of those screened at LCTHC have been diagnosed with Diabetic Retinopathy, Macular Edema, Glaucoma, or Cataracts.

  9. HEALTH SCREENINGS • Monthly Screens: • Robinson • Big Valley • Scotts Valley • Upper Lake • Elem • Other Locations: • WIC • Tribal TANF • Local Community Events • Out-of-County Tribal Agencies and Communities

  10. CAR SEAT & SAFETY EDUCATION • Ferlun/CHR provides car seat safety class, the 2nd and 4th Thursday of each month. • Bike Helmets and Smoke Alarms are also available.

DIABETES EDUCATION CLASSES • Public Health Staff to provide a multifaceted approach to decreasing the complications of Diabetes by offering: • Diabetes Educational Sessions, with new topic each month, topics have included: Heart Health Discussions, Immunizations and Blood Tests, Eye Care, Nutrition Topics, Kidney Care, etc. • Hands-On Cooking Classes • Diabetes Prevention Classes are offered weekly, in the evenings

DIABETES CASE MANAGEMENT • Jacqueline, Diabetes Case Manager, visits each rancheria once a week, providing classes and 1:1 diabetes education. • Joyce, Diabetes Prevention Assistant, sends out reminders to those in the community needing diabetes management labs, eye appointments, foot screenings, etc. • Perform monthly audits, analyzes diabetes statistics and outcomes for the community. • Works closely with Medical Team.

  13. EMERGENCY PREPAREDNESS • Tom/CHR and Disaster Preparedness Coordinator attends local and state emergency preparedness meetings. • Attends Pandemic Flu and other Emergency Preparedness/Communication Drills, representing the tribal community. • Procures/maintains emergency/disaster supplies for the community • Prepares mass flu clinics for all rancherias, as well as, successful drive-thru flu clinics at LCTHC.

  14. COMMUNITY EVENTS • DIABETES PREVENTION HEALTH FAIR- March 2011 The 1st Annual Diabetes Prevention Health Fair was excellent. Over 150 attended, and 90 were screened for pre-diabetes. The Health Fair offered blood pressure, A1c, and many educational venues. • TRIBAL OLYMPICS- June 2011 An excellent program for young children to exercise in friendly competition with local rancherias. A huge event offering good food, gathering of tribes, a fun day for the family! Over 400 gathered in 2011 for the event. • TULE BOAT RACES- July 2011 Public Health staff provide blood pressure and blood sugar screenings, health education, and a healthy snack for 2 day event. • FLU CLINICS- Fall 2011 Each year flu clinics are offered at each rancheria, in addition to flu shots for the elderly/disabled per home visit. Over 350 were immunized last fall. Plus, we launched a successful Drive Thru Flu Clinic, for the 2nd straight year! • ELDER DINNER DANCES- Quarterly throughout the year Quarterly dances provide emotional support and well-being to the native community through social gatherings and dance.

  15. SUMMARY • The Public Health and Diabetes Department works to support the mission statement of Lake County Tribal Health by: Improving the physical, spiritual, emotional and social status of Native Americans through culturally sensitive health care services.
