  2. PCN Agency Role & Expectations What is PCN? • PCN serves individuals who are experiencing homelessness or at-risk individuals • PCN aims to form partnerships between community volunteers and non-profits, government, and private sector to provide services and resources during a one-day event

  3. In 2004, the San Francisco Department of Public Health created Project Homeless Connect • In 2009, Project Homeless Connect Buffalo was launched • Connected over 2,500 homeless and at-risk individuals with hundreds of free services • Distributed more than 3,000 gift cards • Distributed more than 10,000 personal care items PCN Agency Role & Expectations History

PCN Agency Role & Expectations PCN Origination • Debrief from the 2015 Niagara County Poverty Conference • How to directly address and impact poverty in Niagara County? • 25.3% of people in Niagara Falls and 13.6% of people in Niagara County live below the poverty line • Most homelessness originated in the City of Niagara Falls, with 14301 being the most frequently cited last address • 1817 individuals identify as homeless in Niagara County (25% of WNY homeless population) • Formation of a sub-committee • Project Connect Niagara Planning Committee

Over 250 individuals served annually • About 120 individuals receive medical and dental referrals each year • Over 300: • lunches provided • gift cards ($5) provided • Over 220 personal care bags • Over 2,200 items • In-Kind/Pro-Bono Support • Space • Transportation • Personal Care Items PCN Agency Role & Expectations Impact to Date Over 70 organizations have participated (about 50 annually) 50 volunteers annually 245 hours served each year

PCN Agency Role & Expectations Prior to PCN • Prior to PCN, all participating agencies must submit a copy of their liability insurance listing Heart, Love and Soul Inc. as additionally insured • You can mail a copy to P.O. Box 1930, Niagara University, N.Y. 14109, email projectconnectniagara@gmail.com, or upload the form upon registratio • All service providers will be provided with a 4-foot table for this event

PCN Agency Role & Expectations Registration & Set-Up • Please register the morning of PCN (Wednesday, July 24) at the agency registration table outside in front of Heart Love & Soul • Agency Registration will open at 8:00am • Each agency will receive the following: • ID Tags • Floor Plan • Food Voucher • Hole Puncher

  9. PCN Agency Role & Expectations Registration & Set-Up • All agencies are required to be set up and present at their tables by 8:30am

  10. PCN Agency Role & Expectations Registration & Set-Up • PCN has been advertised to take place from 9:00am – 1:30pm • We ask that agencies be prepared to stay until 2:00pm in case of latecomers

  11. PCN Agency Role & Expectations Items Provided • ID Tags • Each agency will be provided with ID tags • Floor Plan • Each agency will be provided with a floor plan • Food Voucher • Each agency will be provided with food voucher(s)

PCN Agency Role & Expectations Items Provided Continued • Hole Puncher • Each agency will be provided a hole puncher • When you discuss your agency’s services with a client we ask that you use the hole punch provided to punch one single hole in that client’s card • Clients are provided punch card during their intake survey • Clients are instructed that if they meet with 8 service providers they will qualify for a free giveaway • Note: PCN uses this incentive to encourage clients to learn about services they might not otherwise access • Please do not punch their card before clients hear about your organization or provide multiple punches to the same client

PCN Agency Role & Expectations Volunteers • PCN Volunteers • All PCN volunteers will be wearing t-shirts with the Project Connect logo • Volunteer Navigators and Event Support Volunteers will have specific roles in assisting each client to connect with various services and resources available at PCN
