Good Practice • All commissioned services accept referrals via one route • SPE hosted by Coventry and Warwickshire Partnership NHS Trust. • Referrals screened by clinician/practitioners who make a decision re most appropriate service across tiers • Clinicians/practitioners across 4 tiers • Self referrals are accepted • Professional referrals are accepted • Step up and down across tiers Coventry & Warwickshire Mind Coventry Relate & Rugby & NE Warwickshire Relate

Integrated Primary Mental Health Service Aims…… • To build capability with Universal ( Tier 1 ) Services • Help professionals recognise signs and symptoms of poor mental health • Support professionals in engaging with children and young people • Support professionals with joint assessments • Early intervention • Supporting signposting to Specialist CAMHS and Targeted Services

  8. Targeted support for Looked After (LAC) adopted children and young people, their parents/carers, and associated professionals (e.g. Social Workers). Services and support for children and young people with low to moderate emotional wellbeing. Targeted tier 2 Service – Vulnerable/at risk Children

  10. Reach Model • Counselling: Reach will provide counselling to c&yp using their preferred method of delivered. Either Face to Face or via • Therapeutic Group Work: Programmes include; Low Mood, Anger Awareness, Anxiety Management, Separation & Loss and Bereavement. Peer support: Opportunity for c&yp to manage their issues through peer support via: Online forums & Group sessions • Self-help resources / informationSignposting and linking to other services • Online self-help resources • Online information Coventry & Warwickshire Mind Coventry Relate & Rugby & NE Warwickshire Relate

The staff team • Journeys and Reach employ a team with a wide range of skills and qualifications including: • Primary Mental Health Workers • Occupational Therapists • Counsellors • Youth workers • NNEB • Psychologists (outsourced) Coventry & Warwickshire Mind Coventry Relate & Rugby & NE Warwickshire Relate

Benefits of Partnership Working For children, young people, parents and professionals • More choice • Diverse range of specialism's, modalities, knowledge and experience • Smoother transition between services • Joint working • Easier access into the most suitable services without having to retell their story • Preventing delays in accessing the right service first time • Reduced waiting times

Access to support before crisis point – making sure people with mental health problems can get help 24 hours a day and that when they ask for help, they are taken seriously. • Urgent and emergency access to crisis care – making sure that a mental health crisis is treated with the same urgency as a physical health emergency. • Quality of treatment and care when in crisis – making sure that people are treated with dignity and respect, in a therapeutic environment. • Recovery and staying well – preventing future crises by making sure people are referred to appropriate services. Let’s not forget our Children & Young People when developing our action plans
