  1. Vaccination Compliance Jump Start! Your guide to navigating your first year of Veterinary School

Vaccine Requirements for Year 1 • Tetanus-Diphtheria-PertussisOne (1) dose of TDap received within the past ten (10) years OR one (1) dose of TDap received since age 11 and one(1) dose or either TDap or TD within the last ten (10) years. • Tuberculosis TestA Tuberculin skin test OR Interferon Gamma Release Assay (IGRA) blood test must be completed no more than six (6) months prior to the semester start date. • Rabies Vaccination Series More details are available on the following slides. Rabies Titers will be required before students enter into the clinical year.

  3. Rabies vaccination seriesRequired for ALL students • It is a requirement for all Vet Med students to have their Rabies series completed by the end of Autumn semester. This is a series of three shots that must be given according to the following schedule: • . • The first shot • Day 7: the second shot • Day 21 – 28: the third shot • . • You can complete this requirement over the summer while you are at home and can visit your family doctor, or you can make appointments with the Wilce Student Health Center on campus.

  4. Rabies vaccination seriesRequired for ALL students • For making an appointment at the Wilce Student Health Center: • *If you plan to use private third party insurance, you can make an appointment through the Wilce Student Health Center anytime after you are admitted. • *If you plan to use the OSU Student Health Insurance plan, you will need to wait until it is one week before class starts to make an appointment for the three dose vaccine series. Students can call 614-292-4321 to schedule an appointment at the Wilce Student Health Center on campus

Additional Required Vaccinations • Adult Tetanus/Diphtheria/Pertussis (Tdap) vaccine • Must be given within the last 10 years • If expired, a TD booster is acceptable • One single Tuberculosis skin test (PPD) in your first year of the program • This is not an annual requirement • One single test completed within 6 months of program start date is required • Please call 614-292-4321 to schedule an appointment at SHS

  6. College of Vet Med Compliance • All vaccines and testing for Vet Med compliance must be submitted to the Preventive Medicine department at Student Health Services by the end of your first semester • Email: preventivemedicine@osu.edu • Fax: 614-292-6001 • Upload: MyBuckMD via shs.osu.edu

The Ohio State University Vaccination Requirement • It is mandatory that students new to The Ohio State University complete their general population vaccination requirementsin addition to the ones required by the College of Vet Med. • . • The Ohio State University vaccination requirements must be completed prior to the start of the semester in which the To-Do List item is assigned. • You can view your To-Do List in your Student Center (BuckeyeLink) . • Submission for the OSU Vaccination Requirement DOES NOT satisfy your College of Vet Med requirements. You must submit to both departments to become compliant for both requirements. Follow the submission instructions on the previous and following pages.

  8. OSU Vaccination Requirement • Hepatitis B • MMR • Tdap • Varicella • Meningococcal (if under 22 years old) • Polio (if under 18 years old) • Tuberculosis (if an international student) • Forms and instructions can be found at shs.osu.edu -> Services -> Vaccinations -> Vaccination Requirement

OSU Vaccination Requirement: Step 1 • Completion of a vaccination history signed by a licensed medical provider. • 4 ways to complete this step: • Upload the record through the “Document upload” feature in your MYBUCKMD portal at shs.osu.edu. • You can log on to MyBuckMD with your university name.# and password • Attach the record in an email to vaccination@osu.edu • Fax the record to 614-292-7042 • Drop off your record to room 264C at the Wilce Student Health Center

OSU Vaccination Requirement: Step 2 • Completion of an electronic form for direct data entry. • The step 2 electronic form can be found under the “My Forms” section of your MyBUCKMD web portal (shs.osu.edu). • Locate the Vaccination Requirement electronic form • Use the dates from the document in step 1 (signed vaccination history) to successfully complete the electronic form (see below).
