Make sure certain services are covered in advance • Services that need prior authorization include: • Hospitalizations • Transplants and transplant-related services • Behavioral health stays • Home health care • Infertility treatment • Durable medical equipment • Over $500 for rental • Over $1,500 for purchase • Over $1,000 for prosthetics • Clinical trials (QHDHP only)
