  2. One year ago, our members voted to approve the merger of Connecticut AIDS Resource Coalition with AIDS Project Hartford. On January 1, 2013, the merger became official and AIDS Connecticut was created. The past year has brought many changes, both big and small…

  3. A New Mission: AIDS Connecticut (ACT), in partnership with its member agencies, improves the lives of people impacted by HIV through care and supportive services, housing, advocacy and prevention.

  6. The programs provided by CARC have been combined with those offered by AIDS Project Hartford to create a full complement of services that will be used to more efficiently support Connecticut’s HIV/AIDS community. These newly merged services include…

  7. Financial Assistance ACT assists families and individuals with obtaining and maintaining housing through the Housing Assistance Fund and the Emergency Housing Fund; and meeting emergency expenses through the Client Assistance Fund.

  8. Care Services ACT provides the following care services for people living with HIV/AIDS in the Hartford area: Medical Case Management, Project TLC, Housing Support and Medication Adherence.

  9. Connections Wellness Center • Connections is an HIV drop-in center that offers services including: • Medical case management • Support groups • Meals • Substance abuse counseling • Mental health counseling • Acupuncture • Research studies • Social activities

  10. AIDS LIFE Campaign The AIDS LIFE (Legislative Initiative and Funding Effort) Campaign is Connecticut's only statewide group that focuses solely on all of the policy-related issues impacting people living with and at high risk for HIV/AIDS.

  11. Case Management Training Institute CMTI is a comprehensive training program designed for those with case management responsibilities in AIDS services, AIDS housing, domestic violence, shelters and transitional living programs.

  12. Prevention & Outreach ACT provides Comprehensive Risk Counseling Services (CRCS), Counseling & Testing, and partners with the faith community and Healthy Teens CT.

  13. Hartford Syringe Exchange The Syringe Exchange Program is a harm-reduction service that encourages injection drug users to dispose of used hypodermic needles rather than sharing the needles and increasing the transmission of blood borne pathogens.

  14. CT Working Positive Project ACT seeks to provide cross training between employment services and all types of AIDS service providers, so that they can better assist persons living with AIDS who seek to (re)enter the workforce.

  15. Homeless Management Information System (HMIS) ACT is a close partner with the Connecticut Coalition to End Homelessness in the quest to gather and maintain accurate information about homelessness in the state of Connecticut.

  16. nPEP - HIV Prevention for Sexual Assault Survivors ACT has been funded by the CT Department of Public Health to establish a program that will pay for Non-occupational Post-Exposure Prophylaxis (nPEP) for survivors of sexual assault who are un- or under-insured.

  17. Technical Assistance & Networking ACT provides many technical assistance options and networking opportunities for our member agencies, funders and consumers.

  18. Quality Assurance ACT conducts annual quality assurance audits on all AIDS housing programs in Connecticut. Results are used to ensure individual program compliance as well as to inform members of best practices, trends, and systemic challenges and barriers.
