Background • The incidence of irregular allo-antibodies (IAA) against red blood cells (RBC) in general population is unknown. • The question is relevant during overseas military operations since RBC are transfused without awareness of the presence of such antibodies, so that one asks if a IAA screening should be conducted in every servicemen before overseas operations.

  3. Methods • A research of IAA was performed in all blood donations in the French armies national blood bank during the year 2007. • The incidence IAA was analysed according to age and gender of donors

  4. Results (1) 17 784 donations 44 IAA + IAA clinically relevant:11 (11 donors)mean age 32 (21-47) No incidence on transfusion policy: 33/44 6.2/10 000 (CI 95% : 3.1 –11/10 000) 8 women 3 men 14.5/10 000 2.4/10 000 p=0.0001

  5. Results (2) • anti-RHD (anti-RH1) 4/11anti-E (anti-RH3) 7/11anti-c (anti-RH4) 2/11anti-S (anti-MNS3) 1/11 • Three women had two different antibodies for each: anti-D+E or anti-E+c or anti-c+S • Anti-E antibodies identified in men are natural antibodies probably without risk of adverse reaction.

  6. Conclusion • According to these data (rarity of relevant antibodies and low benefit/risk ratio) the French army medical service has chosen to not perform mandatory IAA screening for servicemen
