Graduation Transitions Graduation Transitions is an independently completed course (4 credits) that is REQUIRED for graduation in BC. • You will need to READ ALL INSTRUCTIONS and refer to all HANDOUTS/EXAMPLES/WEBSITES provided in order to complete this course • All components of the 9 page booklet (5 main sections) need to be FULLY COMPLETED in order receive credit for the program • QUALITY is essential, without it, you will not pass ** YOUR FULLY COMPLETED GRAD TRANS is DUE ON FRIDAY, DECEMBER 12, 2014 ** • Your counsellor will meet with you after this date to discuss your completed binder and will assess whether or not you have met the requirement.

  3. Where Can I Find It? • The updated Grad Transitions booklet for 2014-2015 is now available online on the ND Website: http://nd.deltasd.bc.ca • From the ND homepage, follow these steps: • Click on Students • Click on Grads • Click on Grad Transitions

  4. What Do I Need To Do? Once on the Grad Transitions webpage, get started by doing the following: • Print out a hard copy of the full (9 page) Grad Transitions Booklet. • Write your First/Last name and Due Date clearly on the front cover of the booklet. • Print off one copy of each of the (8) Required Handouts. • Have a look at each of the (7) Useful Websites to get a sense of what info you will find there.   5. Now Get It Done!! The Graduation Transitions Program is made up of Five Main areas of SELF-DIRECTED Self-Exploration: • WHO am I? • WHAT have I learned? • WHAT have I done? • WHERE am I going? • HOW am I going to get there? For each of these FIVE MAIN AREAS, you will be required to show action, evidence, reflection and communication. Just follow the booklet page by page – the instructions and assignments are all there!! You will need to documented each completed item on the checklist in your FINAL GRAD TRANSITIONS BINDER, which is due to your Counsellor by no later than FRIDAY, DECEMBER 12, 2014!!

  6. Checklist for Completion On Page 8 of the Grad Transitions booklet, you will find a checklist of all the items that are necessary to complete the program. 1. WHO AM I? Include write-ups and visual supports for each of the following: 1. Personality Traits 2. Activities you enjoy 3. Strengths 4. Skills 5. Healthy Living Plan (all four areas) 2. WHAT HAVE I LEARNED? 1. Completed Grad Program Requirement page 3. WHAT HAVE I DONE? 1. DPA Report (completed and signed) 2. Three achievements with a reflection for each 3. 30 Hours of Work Experience/Community Service sheet completed and signed. 4. Attach reflection questions answered on a separate page 

  7. Checklist continued… 4. WHERE AM I GOING? 1. A fully developed and supported Transition Plan for the first year after high school 2. A fully developed and supported Back Up Transition Plan for the first year after high school 5. HOW AM I GOING TO GET THERE? 1. Completed application form that matches your plan 2. Completed PSI Selection, with code provided as proof (if studying that first year) 3. Current Resume 4. Cover letter that matches your plan 5. Completed Budget Plan (for school or work/travel) ****Final Submission Date: FRIDAY, DECEMBER 12, 2014 BY 3:00PM**** REVIEW MEETING with your Counsellor to follow: A-G: Mr. McGill H-L: Ms. Gillis M-S: Ms. Kaila T-Z: Ms. Cipriano
