Student Wellness Functional Plan Key Features • Expansion of the USDA school nutrition programs • A more robust commitment to physical education • Improving the nurse to student ratio • Expansion of social work and psychologist intensive programs and services • Integration, coordination and alignment of the departments and programs that are the primary providers of student health and wellness services. • Educating parents and community partners about student wellness and including them as advocates and participants • Expansion of prevention strategies through coordination with Safe Schools/Healthy Students.

  3. Coordinated School Health • Not a separate program • It is a Comprehensive Model for a School Community • Networking opportunities within the district, state, and nationally • Align the entire district around student health and wellness • Align the school community with Head Start

  4. Coordinated School Health Team • Not Asking to create a new team in the school • Increase communication in the school community • Decrease overlapping/redundant services • Existing School teams can fulfill the role of the CSHT by expanding their membership

  5. Coordinated School Health Team • Who Should be on the CSHT? • Administrator • Teacher • Nurse • Food Service Manager • Physical Education/Health Teacher • Psychologist • Social Worker • Counselor • Parent • Student

CSHT Assessment Tool • CSHT Assessment Tool was revised with the help of teachers and school staff • Revised tool is designed to capture data not caught in other ways • Assessment data can be used for: • Title IV Applicants • Ed Plans • Grant Applications • Recognition Awards such as the Governors School Health Award • Informing parents and school community • Proposal & Allocation of Title Funding

  7. Forming CSHT • CSHT could be used to do the Title IV 2009-2010 application (due April 24th) • Technical Support session on April 29th at the Title IV Networking Meeting. • MPS Cafeteria • Registration 4:00 - 4:30 p.m. • Sessions 4:30 - 6:30 p.m.

  8. CSHT Assessment Tool • Due Date: June 12th, 2009 • Part of the School Close Out each year

How to fill out the survey • Go to the Wellness & Prevention Office website: www.wellnessandpreventionoffice.org. • Click on the white CSHT Assessment Tool tab at the top of the page on the right side. When you click on the CSHT Assessment Tool it will take you to the instruction page, print a hard copy of the assessment tool to use as a worksheet with your CSHTeam. (See red notation under item # 2 to print hard copy). • Complete and submit only one on-line assessment tool. To save a hard copy of your on-line responses for your records, print each page before you proceed to the next page. • You must identify the CSHT Chairperson and all committee members on the first page. • You must hit Submit at the bottom of the final page before logging off.
