Administrative Requirements Forms due with proposal • Vendor Identification Form • Affidavit of Non-Collusion • Bid Bond • Bidder Contributions CEC Form 55 • Contractor Responsibility Program (Questionnaire and Pledge) • Equal Benefits Ordinance (EBO Affidavit) • Municipal Lobbying Ordinance CEC Form 50 • Small Business Enterprise Program (SBE Affidavit and Subcontractor Participation Plan)

Vendor Identification Form Mandatory form that requires the following information: • Bidder/Proposer’s business activity data • Company’s Equal Employment Opportunity Officer contact information • Business Tax/Vendor Registration Number • List of Other City of Los Angeles Contracts (during previous ten years

  4. Affidavit of Non-Collusion • Genuine proposal, not for comparative purposes • Must be notarized by a registered Notary Public • Must be signed by an authorized company officer Submit Affidavit with the proposal to be considered “responsive”

  5. Bid Bond • Bid Bond in the amount of 10% of the bid • Certified Check from bank within the City of Los Angeles; • Cashier’s Check issued by bank; • Surety Bond (use the attached form) • Bidder notarized signature • Corporate Seal

  6. Bidder Contributions • Certifies that Proposer will refrain from making campaign contributions and/or engage in fundraising activities for LA City elected officials (including City Attorney and Controller), candidates for those offices, and committees they control (effective as of date of proposal submission) • Proposers are required to complete and submit the Bidder Contributions CEC Form 55 with the proposal • Any proposal made without the CEC Form 55 will be considered not responsive

  7. Contractor Responsibility Program • Proposer’s quality, fitness and capacity to perform the work set forth in the contract. • All bidders/proposers are required to complete and submit the Contractor Responsibility Questionnaire and Pledge of Compliance with the bid/proposal. • Disqualification from the bidding process or contract termination for failure to comply

Equal Benefits Ordinance • The EBO requires City contractors who provide benefits to employees with spouses provide the same benefits to employees with domestic partners. • Domestic partners are defined as two adults living together, jointly responsible for living expenses, committed to an intimate and caring relationship and registered as domestic partners with a governmental entity. • Submit EBO Affidavit with the proposal.

  9. Municipal Lobbying Ordinance • Disclosure requirements and prohibitions regarding Proposer’s lobbyist activities • Bidders are required to complete and submit the Municipal Lobbying Ordinance CEC Form 50 with the bid • Any bid made without the CEC Form 50 may be considered not responsive

Small Business Enterprise Program • 15% Mandatory goal established by LAWA – must meet or exceed this goal to be considered responsive • Penalty for failure to meet goal • 100% participation if Prime contractor is a CertifiedSBE • Submit the SBE Affidavit with the proposal • Submit the SBE Subcontractor Participation Plan with the proposal

  11. SBE Certification If your firm is currently certified with one of the following agencies you do NOT need to go through the Certification process, simply submit a copy of your certification with your bid/proposal.

  13. Living Wage and Service Contractor Worker Retention Ordinances • Contractor agrees to pay employees a living wage – updated July 1st of every year. The living wage is currently $11.03 per hour if the contractor pays for health benefits or, if health benefits are not provided, then $15.84 per hour for Airport Employees. • Contractor must provide at least 12 compensated days off per year for each employee’s sick leave, vacation or personal necessity, and at least 10 days off uncompensated. • Access to payroll records in event of audit performed by Public Works, Bureau of Contract Administration.

Insurance • $1,000,000 Automobile Liability • $1,000,000 Aviation/Airport OR General Liability • $1,000,000 Professional Liability • Worker’s Compensation (Statutory) • LAWA must be “additional insured” onto the policy • Coverage amount depends on nature of work to be done. Where required, both prime and subcontractors have to show proof of coverage prior to starting work.

First Source Hiring Program • The First Source Hiring Program (FSHP) provides current and new LAWA contractors with pre-screened and pre-qualified candidates for hiring consideration for non-trade positions at LAX. • Positions include: management, professional, clerical, food service, retail, security, baggage, warehouse, drivers, ramp and ticket agents, etc…. • FSHP language contained in LAWA contracts require employers and their sub-contractors to post open positions on the FSHP Applicant Tracking System (ATS) prior to advertising to the general public.

Benefits for LAX Employers • FSHP saves time and $$$ for employers because candidates are pre-qualified and pre-screened for airport security badging. • The FHSP partners with local WorksourceCenters, colleges, EDD, and other agencies to provide LAX employers with access to thousands of applicants. • The BJRC also provides recruiting, job fairs, and other employment services for participating companies. • FSHP services are provided at NO COSTto your company

  18. Next Steps for Selected Contractors • Designate a liaison to work with the FSHP (preferably Human Resources personnel) and register your company with the FSHP. The New Employer Registration Form is located here: www.lawa.org/bjrc. • Open positions are posted on our website: www.lawa.org/bjrc. • Review candidates in real time and interview FSHP candidates before opening the positions to the general public. • Provide feedback on candidates interviewed and provide feedback to help us improve our process.
